摘要
目的:分析益气活血舒筋方治疗腰椎间盘突出症气虚血瘀证的临床效果。方法:回顾2021年3月—2024年3月于黄石市中医医院就诊的95例腰椎间盘突出症患者的临床资料,按治疗方法分成常规组47例与联合组48例。常规组患者接受牵引、塞来昔布口服等常规西医治疗,联合组在此基础上加用益气活血舒筋方口服治疗,两组均连续治疗2周。观察两组中医证候积分、血清学指标、腰椎活动度、腰椎功能与疼痛评分、临床疗效及不良反应。结果:治疗后,联合组中医证候积分、Osweatry功能障碍指数(ODI)、视觉模拟(Visual Analogue Scale/Score,VAS)疼痛评分均低于常规组(P<0.05);联合组前列腺素E2(PGE2)、5-羟色胺(5-HT)、P物质(SP)、神经肽Y(NPY)水平均低于常规组(P<0.05);联合组日本骨科学会评分标准(JOA)评分及腰椎前屈、侧屈、后伸活动度均高于常规组(P<0.05);联合组总治疗有效率高于常规组(P<0.05);不良反应总发生率两组差异无统计学意义(P>0.05)。结论:益气活血舒筋方可提高腰椎间盘突出症气虚血瘀证患者的治疗效果,降低患者疼痛程度,改善患者腰椎活动度及腰椎功能,且具有较高安全性。
Objective:To analyze the clinical effects of Yiqi Huoxue Shujin Formula in treating lumbar disc herniation with Qi deficiency and blood stasis syndrome.Methods:A retrospective analysis was conducted on the clinical data of 95 patients with lumbar disc herniation who visited Huangshi Traditional Chinese Medicine Hospital from March 2021 to March 2024.Patients were divided into two groups based on treatment methods:the conventional group(47 cases)and the combined group(48 cases).The conventional group received routine Western medical treatments such as traction and oral Celecoxib,while the combined group received Yiqi Huoxue Shujin Formula in addition to the conventional treatments.Both groups were treated for 2 weeks.The following parameters were observed:TCM syndrome scores,serological markers,lumbar mobility,lumbar function,pain scores,clinical efficacy,and adverse reactions.Results:After treatment,the combined group had significantly lower TCM syndrome scores,Oswestry Disability Index(ODI),and Visual Analogue Scale(VAS)pain scores compared to the conventional group(P<0.05).The combined group also had significantly lower levels of prostaglandin E2(PGE2),serotonin(5-HT),substance P(SP),and neuropeptide Y(NPY)compared to the conventional group(P<0.05).Moreover,the combined group showed significantly higher Japanese Orthopedic Association(JOA)scores and better lumbar flexion,side bending,and extension compared to the conventional group(P<0.05).The total clinical efficacy rate in the combined group was higher than in the conventional group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Yiqi Huoxue Shujin Formula can improve the therapeutic effect in patients with lumbar disc herniation and Qi deficiency and blood stasis syndrome,reduce pain,improve lumbar mobility and function,and has a relatively high safety profile.
