摘要
目的:分析针灸结合参芪络痹方治疗脑梗死伴颈动脉粥样硬化(CAS)斑块的临床效果。方法:选取2022年5月—2024年4月江阴市中医院收治的98例脑梗死伴CAS斑块患者,按照不同治疗方法分为对照组和观察组,对照组52例应用针灸治疗,观察组46例另加用参芪络痹方治疗,对比两组临床疗效、中医证候积分、血液流变学指标、颈动脉斑块相关指标、血脂指标,观察两组不良反应发生情况。结果:观察组中医证候积分和神经功能评分较对照组低(P<0.05),观察组治疗有效率较对照组高(P<0.05)。观察组全血高切黏度、低切黏度及纤维蛋白原水平均低于对照组(P<0.05)。观察组内膜中层厚度(IMT)、斑块面积低于对照组(P<0.05),颈动脉管腔直径(CCAD)高于对照组(P<0.05)。观察组不良反应发生率低于对照组,但差异无统计学意义(P>0.05)。治疗后,观察组低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、甘油三酯(TG)水平优于对照组(P<0.05)。结论:针灸结合参芪络痹方治疗脑梗死伴CAS斑块效果显著,可显著减轻中医证候积分,改善神经功能、血脂指标,改善血液流变学和颈动脉斑块相关指标,临床用药安全性尚可。
Objective:To analyze the clinical efficacy of acupuncture and moxibustion combined with Shenqi Luobi Formula in treating cerebral infarction with carotid artery atherosclerosis(CAS)plaques.Methods:A total of 98 patients with cerebral infarction accompanied by CAS plaques who were admitted to Jiangyin Hospital of Traditional Chinese Medicine from May 2022 to April 2024 were selected.They were randomly divided into two groups:the control group(52 patients),who received acupuncture and moxibustion treatment,and the observation group(46 patients),who received acupuncture and moxibustion combined with Shenqi Luobi Formula.Clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,hemorheological parameters,carotid plaque-related indicators,lipid profile,and adverse reaction rates were compared between the two groups.Results:The TCM syndrome scores and neurological function scores in the observation group were lower than those in the control group(P<0.05).The treatment efficacy rate in the observation group was higher than that in the control group(P<0.05).Hemorheological parameters such as whole blood high shear/low shear viscosity and fibrinogen levels were lower in the observation group(P<0.05).The intima-media thickness(IMT),plaque area,and carotid artery cross-sectional area diameter(CCAD)were lower in the observation group(P<0.05).The incidence of adverse reactions in the observation group was lower than in the control group(P>0.05).After treatment,the observation group showed better lipid profile parameters,including lower levels of low-density lipoprotein cholesterol(LDL-C),higher levels of high-density lipoprotein cholesterol(HDL-C),total cholesterol(TC),and triglycerides(TG)compared to the control group(P<0.05).Conclusion:Acupuncture and moxibustion combined with Shenqi Luobi Formula is effective in treating cerebral infarction with CAS plaques.It can significantly reduce TCM syndrome scores,improve neurological function,lipid profile,hemorheological parameters,and carotid plaque-related indicators.The clinical safety of the treatment is acceptable.
