摘要
目的探讨Teach-back健康教育模式在慢性心力衰竭患者低钠饮食管理中的应用效果。方法选择2022年10月—2023年9月江西省某医院心内科收治的114例慢性心力衰竭患者为研究对象,按照组间基本特征具有可比性的原则分为对照组和观察组,各57例。对照组采取常规护理干预实施低钠饮食管理,观察组应用Teach-back健康教育模式进行低钠饮食管理,两组干预期为3个月。比较两组患者干预前后的低钠饮食管理依从性和行为[低钠饮食问卷(DSRQ)]、自我管理能力(慢性病管理自我效能量表)及心力衰竭生活质量[明尼苏达心衰生活质量问卷(MLHFQ)]。结果干预前,两组患者低钠饮食管理依从性和行为、自我管理能力及心力衰竭生活质量比较差异均无统计学意义(P>0.05);干预后,观察组患者低钠饮食管理依从性和行为、自我管理能力的各维度评分及总分高于对照组,组间比较差异有统计学意义(P<0.05);心力衰竭生活质量各维度评分及总分低于对照组,组间比较差异有统计学意义(P<0.05)。结论应用Teach-back健康教育模式对慢性心力衰竭患者进行低钠饮食管理,可提高其低钠饮食知识及依从性,形成良好饮食行为,提高疾病自我管理能力并改善生活质量。
Objective To explore the application effect of the Teach-back health education model in low-sodium diet management for patients with chronic heart failure.Methods A total of 114 patients with chronic heart failure admitted to the Department of Cardiology of a hospital in Jiangxi Province from October 2022 to September 2023 were selected as study subjects.According to the principle of comparability of basic characteristics between groups,they were divided into a control group and an observation group,with 57 cases in each group.The control group received routine nursing intervention for low-sodium diet management,while the observation group received the Teach-back health education model for low-sodium diet management.The intervention period for both groups was 3 months.The two groups were compared before and after the intervention in terms of low-sodium diet adherence and behavior[Dietary-Sodium Restriction Questionnaire(DSRQ)],selfmanagement ability(Chronic Disease Management Self-Efficacy Scale),and heart failure-related quality of life[Minnesota Living with Heart Failure Questionnaire(MLHFQ)].Results Before the intervention,there was no statistically significant difference in the low-sodium diet adherence and behavior,self-management ability,and heart failure-related quality of life between the two groups(P>0.05).After intervention,the scores for each dimension and total score of low-sodium diet adherence and behavior,as well as self-management ability were higher in the observation group than those in the control group,the differences between the groups were statistically significant(P<0.05);the scores for each dimension and total score of heart failure-related quality of life were lower in the observation group compared to the control group,the differences between the groups were statistically significant(P<0.05).Conclusion The application of the Teach-back health education model for low-sodium diet management in patients with chronic heart failure can improve their knowledge and adherence to the low-sodium diet,foster healthy dietary behaviors,enhance self-management abilities,and improve quality of life.
