摘要
目的 探讨强直性脊柱炎(ankylosing spondylitis, AS)患者全麻下行侧卧位纤维支气管镜引导经口气管插管术的效果。方法 简单随机选取2021年1月—2023年12月上海交通大学医学院苏州九龙医院收治的84例AS患者为研究对象,根据手术体位不同分为两组,各42例。两组均全麻下行纤维支气管镜引导经口气管插管术,对照组采取仰卧位,观察组采取侧卧位。比较两组气管插管时间、置管时间、首次气管插管成功率、首次置管成功率及不良事件发生情况。结果 观察组气管插管时间、置管时间均短于对照组,差异均有统计学意义(P均<0.05)。观察组首次气管插管成功率、首次置管成功率均高于对照组,差异均有统计学意义(P均<0.05)。观察组组间不良事件发生率为4.76%(2/42),低于对照组的19.05%(8/42),差异有统计学意义(χ^(2)=4.087,P<0.05)。结论 AS患者取侧卧位进行全麻下纤维支气管镜引导经口气管插管,不仅能够显著提高插管成功率,节约操作时间,同时还可提高气管插管安全性。
Objective To investigate the effect of fiberoptic bronchoscopy-guided orotracheal intubation in lateral position under general anesthesia in patients with ankylosing spondylitis(AS).Methods A total of eighty-four AS patients admitted to Suzhou Kowloon Hospital Shanghai Jiao Tong University School of Medicine from January 2021 to December 2023 were simple randomly selected as the study objects and divided into two groups according to different surgical positions,with forty-two cases in each group.Both groups underwent fiberoptic bronchoscopy-guided orotracheal intubation under general anesthesia.The control group was in supine position,and the observation group was in lateral position.The intubation time,intubation time,success rate of first intubation,success rate of first intubation and adverse events were compared between the two groups.Results The tracheal intubation time and catheterization time in the observation group were shorter than those in the control group,and the differences were statistically significant(both P<0.05).The success rate of the first tracheal intubation and the success rate of the first intubation in the observation group were higher than those in the control group,and the differences were statistically significant(both P<0.05).The incidence of adverse events in the observation group was 4.76%(2/42),which was lower than 19.05%(8/42)in the control group,and the difference was statistically significant(χ^(2)=4.087,P<0.05).Conclusion Fiberoptic bronchoscope guided orotracheal intubation under general anesthesia in AS patients in lateral position can not only significantly improve the success rate of intubation,save operation time,but also improve the safety of tracheal intubation.
