摘要
植入剂是一种缓控释给药系统,具有长期持续稳定释药、患者依从性较好、生物利用度高等独特优势,现已在长期止痛、肿瘤治疗、眼部疾病治疗、心血管疾病治疗、生殖健康等领域有着越来越广泛的应用。随着近几年高分子材料的研究和发展,聚乳酸、聚乙醇酸、聚己内酯、聚乳酸-乙醇酸共聚物等有良好生物相容性的高分子聚合物材料逐渐被用于植入剂的制备和生产。研究发现这些聚合物材料的自身特性对植入剂的释药行为有一定影响。本文就植入型给药系统中常用聚合物的种类、降解/释药机制及其影响因素、临床应用实例进行综述,旨在为进一步优化植入剂的控释方式、建立与体内相关性更好的植入剂体外释放方法、推动新型聚合物材料在植入剂中的应用提供参考。
Implantable drug delivery system is a novel type of sustained/controlled-release drug delivery system,offering unique advantages of long-term continuous and stable drug release,good patient compliance,and high bioavailability.Implants have been widely applied in various fields,including long-term pain management,cancer treatment,ophthalmic disease treatment,cardiovascular disease treatment,and reproductive health.With the research and development of polymer materials in recent years,polymers with good biocompatibility,such as polylactic acid,polyglycolic acid,polycaprolactone,and polylactic acid-glycolic acid copolymer,have been gradually used in the preparation and production of implants.Studies indicate that the inherent properties of these polymers have a certain impact on the drug release of implants.In this paper,the types,degradation/drug release mechanisms and their influencing factors,as well as clinical applications of polymers commonly used in implantable systems are reviewed,aiming to provide a reference for further optimizing the sustained/controlled-release mode of implants,establishing in vitro release methods that have better correlation with in vivo conditions for implants,and promoting the application of new polymer materials in implants.
