儿童呼吸系统腺病毒感染流行特征及季节因素分析

摘要 目的:分析儿童呼吸系统腺病毒感染流行特征及季节因素。方法:回顾性分析2019年3月至2023年9月于洛阳石化医院就诊的1012例急性呼吸道感染患儿的临床资料,采用荧光定量PCR(RT-PCR)法对患儿的咽拭子标本进行腺病毒核酸检测,分析腺病毒的流行情况。结果:在1012例急性呼吸道感染的患儿中,HAdV的阳性检出率为14.43%(146/1012),不同性别的患儿HAdV阳性检出率差异无统计学意义(P>0.05),不同年龄段的患儿的HAdV阳性检出率差异有统计学意义(P<0.05),以学龄前儿童的HAdV阳性检出率最高18.57%。不同季节的HAdV阳性检出率差异有统计学意义,以夏季的HAdV阳性检出率最高20.92%(P<0.05)。146例HAdV阳性感染患儿中,单一感染占89.04%(130/146),混合感染占10.27%(15/146)。混合感染以HAdV合并呼吸道合胞病毒感染占比最多(10例,66.66%),其次为HAdV合并甲型流感病毒感染(3例,20.00%)。146例阳性标本检测出C组、B组、E组共7个基因型别,以B组的HAdV检出率最高72.60%,主要为HAdV3型(55例,51.89%)和HAdV7型(22例,20.75%)。结论:HAdV是引起患儿急性呼吸道感染的常见病原体,夏季为HAdV感染的高发季节,好发于学龄前儿童,这提示应加强儿童呼吸道腺病毒感染的防治工作。 Objective:To analyze the epidemiological characteristics and seasonal factors of respiratory adenovirus infection in children.Methods:A retrospective analysis was conducted on the clinical data of 1,012 children with acute respiratory infections who were treated at Luoyang Petrochemical Hospital between March 2019 and September 2023.Adenovirus nucleic acid detection was performed on throat swab samples from the children using real-time fluorescent quantitative PCR(RT-PCR)to analyze the prevalence of adenovirus.Results:Among the 1,012 children with acute respiratory infections,the positive detection rate of human adenovirus(HAdV)was 14.43%(146/1012).There was no significant difference in the positive detection rate of HAdV between children of different genders(P>0.05),but there was a significant difference in the positive detection rate of HAdV among children of different age groups(P<0.05),with preschool children having the highest positive detection rate of HAdV 18.57%.There was a significant difference in the positive detection rate of HAdV in different seasons,with the highest positive detection rate in summer 20.92%(P<0.05).Among the 146 children with HAdV positive infections,single infections accounted for 89.04%(130/146)and mixed infections accounted for 10.27%(15/146).Mixed infections were most commonly HAdV combined with respiratory syncytial virus infection(10 cases,66.66%),followed by HAdV combined with influenza A virus infection(3 cases,20.00%).Seven genotypes from groups C,B,and E were detected in the 146 positive samples,with the highest detection rate of HAdV in group B 72.60%,mainly HAdV3(55 cases,51.89%)and HAdV7(22 cases,20.75%).Conclusion:HAdV is a common pathogen causing acute respiratory infections in children,with summer being the high-incidence season for HAdV infections,especially in preschool children.This suggests that prevention and control efforts for respiratory adenovirus infections in children should be strengthened.
