甲状腺相关眼病患者灰质结构网络拓扑改变及其与焦虑抑郁的相关性研究

摘要 目的研究不同分期甲状腺相关眼病(TAO)患者脑灰质结构网络的拓扑改变及其与焦虑抑郁症状的关联。方法采用回顾性病例对照研究。本研究纳入2019年4月至2020年4月就诊于首都医科大学附属北京友谊医院眼科并进行眼眶MRI检查TAO患者,其中35例非活动期TAO患者(非活动期TAO组)、37例活动期TAO患者(活动期TAO组)以及23名年龄和性别相匹配的健康受试者(健康对照组)作为研究对象。采用结构MRI和基于体素的形态学测量法(VBM)计算灰质体积(GMV),进一步构建形态学灰质脑网络,应用图论分析来评估不同分期TAO患者脑网络全局和节点拓扑属性的改变,及其与患者焦虑、抑郁症状的相关性。结果不同分期TAO组和健康对照组在灰质网络中都表现出高效的小世界属性。与非活动期TAO组和健康对照组相比,活动期TAO组的脑网络全局和局部效率显著下降,最短路径长度显著增加;与健康对照组相比,非活动期TAO组则无显著改变。尽管TAO组与健康对照组的脑网络核心节点分布高度相似,但与健康对照组相比,TAO组的多个脑区的节点介数中心度(BC)显著改变,主要分布在视觉、注意、默认模式网络和感觉运动功能区。活动期TAO组左内侧额上回的节点BC值与汉密尔顿抑郁量表(HAMD)评分呈显著负相关(r=-0.337,P=0.048),左侧岛盖部额下回节点BC值与汉密尔顿焦虑量表(HAMA)评分呈显著正相关(r=0.396,P=0.019)。结论活动期TAO患者情绪-认知功能区的拓扑改变与其焦虑抑郁程度显著相关。 Objective To investigate the disruptions in gray matter structural brain networks in patients with thyroid-associated ophthalmopathy(TAO)at different clinical stages and their association with anxiety and depression symptoms.Methods A retrospective case-control study was conducted.This study included TAO patients who underwent orbital MRI examination in the Department of Ophthalmology,Beijing Friendship Hospital,Capital Medical University from April 2019 to April 2020,including 35 patients with inactive TAO(inactive TAO group),37 patients with active TAO(active TAO group)and 23 age-and sex-matched healthy subjects(healthy control group).Gray matter volume(GMV)was calculated using structural MRI and voxel-based morphometry(VBM),and further morphological gray matter brain networks were constructed.Changes in global and nodal topological properties of brain networks in TAO patients at different stages were evaluated using graph theory analysis,and their correlation with anxiety and depression symptoms was analyzed.Results Both TAO and control groups showed efficient small-world organization in GM networks.However,compared with the inactive TAO group and the healthy control group,the global and local efficiency of the brain network in the active TAO group decreased significantly,and the shortest path length increased significantly.There was no significant change in the inactive TAO group compared with the healthy control group.Although the distribution of brain network core nodes in the TAO group was highly similar to that in the healthy control group,the node betweenness centrality(BC)of multiple brain regions in the TAO group was significantly changed compared with the healthy control group,mainly distributed in the visual,attention,default mode network and sensorimotor functional areas.In the active TAO group,the BC value of the left medial superior frontal gyrus was significantly negatively correlated with the Hamilton depression scale(HAMD)score(r=-0.337,P=0.048),and the BC value of the left inferior frontal gyrus was significantly positively correlated with the Hamilton anxiety scale(HAMA)score(r=0.396,P=0.019).Conclusion The topological changes in the emotional and cognitive functional areas of active TAO patients are significantly correlated with the anxiety and depression severity.
