动脉瘤性蛛网膜下腔出血后早期脑损伤的发生机制

摘要 动脉瘤性蛛网膜下腔出血(aSAH)是一种发病率高、病死率高的脑血管疾病。既往研究认为,aSAH后脑血管痉挛及其引发的迟发性脑缺血是引起患者死亡或残疾的关键因素,但大量研究证实,早期脑损伤可能是引起aSAH不良预后的主要原因。早期脑损伤主要表现为出血后72 h内发生的继发性脑损伤,是一个十分复杂的病理生理变化。本文对aSAH早期脑损伤的发生机制进行综述,以期找到更佳的治疗方法,从而改善患者预后。 Aneurysmal subarachnoid hemorrhage(aSAH)is a cerebrovascular disorder with a high incidence and mortality.Previous studies have postulated that cerebral vasospasm and the resulting delayed cerebral ischemia after aSAH are the crucial factors causing patient death or disability.Nevertheless,a large number of studies have verified that early brain injury might be the primary cause of poor prognosis in aSAH.Early brain injury mainly presents as secondary brain injury occurring within 72 hours after hemorrhage and is an extremely complex pathophysiological alteration.This article conducts a review of the occurrence mechanisms of early brain injury in aSAH,in the hope of finding better therapeutic approaches and thereby improving patient prognosis.
