摘要
目的探究血常规衍生的炎性相关指标与牙周炎的关系。方法本研究采用美国国家健康和营养调查(National Health and Nutrition Examination Survey,NHANES)数据库中2009年至2014年大于45岁的参与者数据,最终纳入5617例。使用加权单变量和多变量logistic回归及受限三次样条回归探讨血常规衍生炎症指标与牙周炎之间的关系。结果Logistic回归分析显示,未调整协变量的情况下,系统性炎症反应指标(systemic inflammatory response index,SIRI)、系统性免疫炎症指标(systemic immune-inflammation index,SII)、中性粒细胞与淋巴细胞比率(neutrophil-to-lymphocyte ratio,NLR)、单核细胞与淋巴细胞比率(monocyte-to-lymphocyte ratio,MLR)与牙周炎呈正相关;调整了全部混杂因素后,SIRI、SII、NLR、MLR仍与牙周炎呈正相关(OR=1.14、OR=1.03、OR=1.10、OR=2.60,P<0.05,)。SIRI、SII、MLR与牙周炎呈线性正相关(非线性P>0.05),NLR与牙周炎是非线性关系(非线性P<0.05)。结论SIRI、SII、MLR、NLR与牙周炎严重程度高度相关,是牙周病预防、诊断、治疗的重要参考指标。
Objective The study aimed to investigate between inflammatory-related indicators derived from blood routine tests and periodontitis.Methods This study employed the data of participants aged over 45 from the National Health and Nutrition Examination Survey(NHANES)database of the United States from 2009 to 2014,and ultimately 5,617 cases were included.Weighted univariate and multivariate logistic regression,as well as restricted cubic spline regression,were employed to explore the relationship between CBC-derived inflammatory markers and periodontitis.Results Logistic regression analysis revealed showed that,without adjusting for covariates,systemic inflammatory response index(SIRI),systemic immune-inflammation index(SII),neutrophil-to-lymphocyte ratio(NLR),and monocyte-to-lymphocyte ratio(MLR)were positively correlated with periodontitis.After adjusting for confounding variables,SIRI,SII,NLR,and MLR remained significantly associated with periodontitis(OR values were 1.14,1.03,1.10,and 2.60,respectively;P<0.05).The fitted smooth curves revealed a linear positive correlation between SIRI,SII,MLR,and periodontitis(non-linear P>0.05),while the relationship between NLR and periodontitis was non-linear(non-linear P<0.05).Conclusion These findings demonstrate that SIRI,SII,NLR,and MLR are associated with an increased risk of periodontitis.These inflammatory indices serve as valuable reference markers for the prevention,diagnosis,and treatment of periodontitis.
