三棱镜联合感知觉训练对间歇性外斜视术后病儿效果及眼血流动力学影响 被引量:1

Effect of triangular prism combined with sensory and perceptual training on postoperative outcome and ocular hemodynamics in children with intermittent exotropia

摘要 目的探讨三棱镜联合感知觉训练对间歇性外斜视(IXT)术后病儿的效果及眼血流动力学的影响。方法选取2018年2月—2023年1月收治的IXT病儿300例,根据治疗方案不同分为对照组和观察组,各150例。对照组给予眼感知觉训练,观察组在对照组基础上联合三棱镜治疗。比较两组临床疗效、斜视棱镜度与融合范围、患眼眼动脉血流动力学指标(收缩期血流速度峰值(Vs)、舒张末期血流速度(Vd)、阻力指数(RI))以及治疗期间不适症状发生情况等指标的差异。结果观察组治疗后总有效率(92.00%)高于对照组(70.00%)(χ^(2)=23.587,P<0.01)。观察组斜视棱镜度-看远、斜视棱镜度-看近显著低于对照组,而融合范围高于对照组(t=9.943~16.149,P<0.01)。治疗后两组病儿的眼动脉Vs、Vd和RI较治疗前显著升高,且观察组的Vs、Vd、RI治疗前后差值均显著高于对照组,差异均有统计学意义(t=4.427~14.538,P<0.01)。观察组治疗后视觉不适症状总发生率低于对照组(χ^(2)=14.584,P<0.01)。结论在维持IXT术后病儿临床疗效和眼血流动力学方面,采用三棱镜联合感知觉训练效果优于单纯眼感知觉训练。 Objective To investigate the effect of triangular prism combined with sensory and perceptual training on posto-perative outcome and ocular hemodynamics in children with intermittent exotropia(IXT).Methods A total of 300 children with IXT who were admitted from February 2018 to January 2023 were enrolled,and according to the treatment regimen,they were divided into observation group and control group,with 150 patients in each group.The children in the control group were given ocular sensory and perceptual training,and those in the observation group were given triangular prism treatment in addition to the treatment in the control group.The two groups were compared in terms of clinical outcome,strabismus prism degree and fusion range,hemodynamic parameters of ophthalmic arteries in the affected eyes(peak systolic velocity(Vs),end-diastolic velocity(Vd),and resistance index(RI)),and the incidence rate of uncomfortable symptoms during treatment.Results The observation group had a significantly higher overall response rate than the control group after treatment(92.00%vs 70.00%,χ^(2)=23.587,P<0.01).Compared with the control group,the observation group had significantly lower values of strabismus prism degree-viewing distance and strabismus prism degree-viewing near and a significantly higher fusion range(t=9.943-16.149,P<0.01).After treatment,both groups had significant increases in Vs,Vd,and RI of ophthalmic arteries,and the observation group had significantly greater increases than the control group(t=4.427-14.538,P<0.01).The observation group had a significantly lower overall incidence rate of visual discomfort symptoms after treatment than the control group(χ^(2)=14.584,P<0.01).Conclusion Triangular prism combined with sensory and perceptual training has a better effect than sensory and perceptual training alone in maintaining postoperative clinical outcome and ocular hemodynamics in patients with IXT.
