摘要
目的目的分析某院住院患者重点监控药品用药医嘱合理性,为下一步开展重点监控药品监管、促进临床合理用药提供参考依据。方法方法从医院HIS系统中等间距调取2023年7月-2023年12月出院患者重点监控药品使用病历,共4020份病历,对病历进行用药合理性专项点评,分析不合理病历及问题原因。结果结果纳入专项点评的4020份病历中有167份不合理用药医嘱,不合理率为4.15%,重点监控药品使用合格率为95.85%。重点监控药品使用合格率逐步升高,医嘱不合理率连续下降。结论结论重点监控药品的使用逐渐趋于合理化,但部分重点监控药品仍需关注其用药合理性。
Objective To analyze the reasonableness of medication orders for key monitored drugs for inpatients in a hospital,and to provide reference for the next step to carry out key monitored drugs supervision and promote clinical rational drug use.Methods A total of 4020 medical records of key monitored drug use of discharged patients from July 2023 to December 2023 were collected from HIS system at a medium interval of the hospital,and special comments were made on the reasonableness of drug use in the medical records,and unreasonable medical records and causes of problems were analyzed.Results Among 4020 medical records included in the special review,167 were irrational drug use,the irrational rate was 4.15%,and the qualified rate of key monitoring drugs was 95.85%.The qualified rate of the use of key monitoring drugs gradually increased,and the unreasonable rate of medical orders continuously decreased.Conclusion The use of key monitoring drugs tends to be rationalized gradually,but some of the key monitoring drugs still need to pay attention to their rationality.
