摘要
目的探讨全身免疫炎症指数(SII)和C反应蛋白与白蛋白比值(CAR)在儿童肺炎支原体肺炎(MPP)并发支气管黏液栓(BMP)中的预测价值。方法选取2023年1月至2024年4月在南京医科大学附属淮安第一医院住院接受支气管镜检查的147例MPP患儿,根据气管镜下是否形成黏液栓分为黏液栓组(74例)和非黏液栓组(73例)。比较两组一般资料及血液检查结果,通过二元logistic逐步回归分析MPP患儿并发BMP的影响因素,并利用受试者工作特征(ROC)曲线评估SII和CAR对MPP患儿并发BMP的预测效能。结果两组热程、热峰、中性粒细胞计数、淋巴细胞计数、C反应蛋白、白蛋白、乳酸脱氢酶、SII、CAR、白细胞介素(IL)-6、IL-10、γ干扰素比较,差异有统计学意义(P<0.05)。二元logistic逐步回归分析显示,SII、CAR为MPP患儿并发BMP的独立危险因素(OR=1.003、1.054,P<0.01)。SII和CAR联合预测MPP患儿并发BMP的曲线下面积高于各指标单独预测。结论SII和CAR在预测MPP患儿并发BMP中具有较高的应用价值。
Objective To explore the predictive value of systemic immune-inflammation index(SII)and C-reactive protein to albumin ratio(CAR)in children with Mycoplasma pneumoniae pneumonia(MPP)complicated with bronchial mucus plugs(BMP).Methods A total of 147 children with MPP who were hospitalized and underwent bronchoscopy at the Affiliated Huaian No.1 People’s Hospital of Nanjing Medical University from January 2023 to April 2024 were selected.According to whether mucus plugs were formed under bronchoscopy,they were divided into the mucus plug group(74 cases)and the non-mucus plug group(73 cases).The general data and blood examination results in two groups were compared;the influencing factors of children with MPP complicated with BMP were analyzed through binary logistic stepwise regression,and the predictive efficacy of SII and CAR for children with MPP complicated with BMP was evaluated by using receiver operating characteristic(ROC)curve.Results There were significant differences in fever course,peak fever,neutrophil count,lymphocyte count,C-reactive protein,albumin,lactate dehydrogenase,SII,CAR,interleukin(IL)-6,IL-10,and interferon-γbetween two groups(P<0.05).Binary logistic stepwise regression analysis showed that SII and CAR were independent risk factors for children with MPP complicated with BMP(OR=1.003,1.054,P<0.01).The area under the curve of SII and CAR combined prediction for children with MPP complicated with BMP was higher than that predicted by each index alone.Conclusion SII and CAR have high application value in predicting children with MPP complicated with BMP.
