摘要
目的:对比单侧双通道内镜手术(UBE)与显微内镜下椎间盘切除术(MED)在单节段腰椎管狭窄症患者中的应用效果。方法:选取2022年1月—2024年1月武警广东总队医院收治的单节段腰椎管狭窄症患者100例作为研究对象,随机分为对照组和观察组,各50例。对照组实施MED,观察组实施UBE。比较两组治疗效果。结果:观察组手术时间短于对照组,术后下床活动时间早于对照组(P<0.05)。术后3d、7d,观察组疼痛评分低于对照组(P<0.05)。观察组并发症总发生率低于对照组(P=0.014)。术后,两组生理健康、心理健康评分均高于术前(P<0.05),且观察组高于对照组(P<0.001)。结论:相较于MED,UBE在单节段腰椎管狭窄症的治疗中具有手术时间短、术后恢复快、术后疼痛程度轻、并发症少、术后生活质量高等优势,值得在临床中推广。
Objective:To compare the application effects of unilateral dual-channel endoscopic surgery(UBE)and microscopic endoscopic discectomy(MED)in patients with single-segment lumbar spinal stenosis.Methods:One hundred patients with single-segment lumbar spinal stenosis admitted to Guangdong Armed Police Corps Hospital from January 2022 to January 2024 were selected as the research objects and randomly divided into control group and observation group,with fifty cases in each group.The control group received MED,while the observation group received UBE.The therapeutic effects of the two groups were compared.Results:The operation time of the observation group was shorter than that of the control group,and the time to get out of bed and move around after the operation was earlier than that of the control group(P<0.05).At 3 days and 7 days after the operation,the pain score of the observation group was lower than that of the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P=0.014).After the operation,the scores of physical health and mental health in both groups were higher than those before the operation(P<0.05),and the scores in the observation group were higher than those in the control group(P<0.001).Conclusion:Compared with MED,UBE has advantages such as shorter operation time,faster postoperative recovery,less postoperative pain,fewer complications and higher postoperative quality of life in the treatment of single-segment lumbar spinal stenosis.It is worthy of promotion in clinical practice.
