摘要
目的探讨在输卵管妊娠的腹腔镜手术治疗中应用输卵管开窗术与切除术的价值。方法选取2022年1月—2023年5月在本院收治的输卵管妊娠患者60例,均行腹腔镜手术,根据所用手术方案将其分为两组,每组30例。开窗组采用输卵管开窗术,切除组采用输卵管切除术,比较两组治疗效果。结果两组术后血清β-绒毛膜促性腺激素(β-HCG)恢复正常时间、住院时间比较,差异无统计学意义(P>0.05);开窗组术中出血量较切除组大,手术时间较切除组更长(P<0.05);切除组术后并发症发生率与开窗组比较,差异无统计学意义(P>0.05);经过12个月的随访,开窗组生育功能与切除组比较,差异无统计学意义(P>0.05)。结论腹腔镜下输卵管开窗术与切除术治疗输卵管妊娠均有良好效果,且安全性高,对生育功能的影响相当,但切除术创伤更小,临床可根据患者实际情况选择手术方案。
Objective To explore the effects of laparoscopic salpingostomy and resection on the treatment of tubal pregnancy and their impact on fertility.Methods From January 2022 to May 2023,60 patients with tubal pregnancy admitted to our hospital underwent laparoscopic surgery.According to different surgical plans,they were divided into fenestration group(30 cases)and resection group(30 cases),and the treatment of the two groups was compared.Results There was no significant difference in the recovery time and hospitalization time of serumβ-chorionic gonadotropin(β-HCG)between the two groups after surgery(P>0.05).The bleeding volume during surgery was greater in the fenestration group than in the resection group,and the surgery time was longer(P<0.05);There was no statistically significant difference in the incidence of postoperative complications between the resection group and the fenestration group(P>0.05);After a 12 months follow up,there was no statistically significant difference in the proportion of intrauterine pregnancies between the fenestration group and the resection group(P>0.05).Conclusion Laparoscopic tubal fenestration and resection have good effects and high safety in the treatment of tubal pregnancy,and have a similar impact on fertility function.However,resection surgery has less trauma,and the surgical plan can be selected according to the actual situation of the patient in clinical practice.
