摘要
未分化多形性肉瘤(undifferentiated pleomorphic sarcoma,UPS)是一种罕见的缺乏谱系特异性分化具有多形性梭形细胞组织学特征的软组织肉瘤。在所有原发性恶性骨肿瘤中<2%,多见于中老年人,通常发生在下肢长骨,特别是股骨,其次是胫骨和肱骨,转移率至少为50%,特别是肺。
Objective To summarize the etiological mechanism,clinical manifestations,pathological manifestations,diagnostic methods,differential diagnosis and treatment of undifferentiated pleomorphic sarcoma through clinical treatment and related literature analysis,and to provide treatment ideas for clinical diagnosis and treatment.Methods The diagnosis,clinical manifestations,differential diagnosis and treatment of a patient with undifferentiated pleomorphic sarcoma in our department were recorded,and relevant literature was screened and analyzed.Results The male patient was 47 years old,and was diagnosed as undifferentiated pleomorphic sarcoma through preoperative puncture biopsy.After clear diagnosis and evaluation of the patient's condition,one cycle of neoadjuvant chemotherapy was given to the patient,followed by resection of femur bone tumor supplemented with individualized total femur and knee artificial prosthesis replacement,and four cycles of neoadjuvant chemotherapy were given to the patient after surgery.The patient's incision recovered well without any other obvious discomfort.No dislocation or breakage of the prosthesis was seen.Moderate functional exercises were carried out,and the follow-up continued.Conclusions Undifferentiated pleomorphic sarcoma can be treated by artificial prosthesis replacement combined with neoadjuvant chemotherapy,which can control the recurrence and metastasis,helping the patient to recover well.
