摘要
目的:分析青少年骨性Ⅱ类下颌后缩患者应用隐形矫治器前导下颌的临床疗效。方法:选取2021年4月-2023年5月笔者医院收治的60例青少年骨性Ⅱ类下颌后缩患者,使用隐形MA矫治器对患者下颌后缩进行矫治。矫治前与导下颌向前后行头颅定位侧位片拍摄,测量影像图中牙骨肌咬合变化,分析治疗前后颌骨与牙齿变化差异。结果:矫治后,患者下颌骨明显前移,颌面部组织形态得到改善。矫治前后患者上牙槽座夹角差异无统计学意义(P>0.05),矫治后患者下牙槽座、上下牙槽座夹角优于矫治前,全面高度、面中高度、面下高度大于矫治前(P<0.05)。矫治后患者咬肌前界与水平面、最大咬合力方向与咬肌前界夹角大于矫治前(P<0.05)。矫治前后患者牙长轴与水平面、牙长轴与牙合平面、后牙牙合平面与水平面夹角差异无统计学意义(P>0.05),矫治后患者牙合平面与水平面夹角大于矫治前,下前牙倾斜度高于矫治前(P<0.05)。结论:隐形矫治器能促进下颌发育,矫治下颌后缩,改善患者侧貌,维持下前牙唇倾度与下颌平面角。
Objective To explore the clinical effect of invisible aligner mandibular advancement(MA)appliance in the adolescents with skeletal Class II malocclusion due to mandibular retrusion.Methods Sixty adolescent patients with skeletal Class II malocclusion due to mandibular retrusion admitted to the hospital from April 2021 to May 2023 were selected,all patients were treated with invisible MA appliance.Lateral cephalometric positioning films were taken before orthodontic treatment.The changes of joint,muscle,and occlusal related indexes were measured,then the temporo-mandibular joint level was compared before and after treatment.Results After orthodontic treatment,there was a forward movement of the mandible,which contributed to excellent improvement of the maxillofacial skeleton and facial soft-tissue morphology.The sella-nasion-A point angle(SNA)yielded no statistical difference between two groups before and after treatment(P>0.05).After treatment,there was an improvement in sella-nasion-B point angle(SNB)and A point-nasion-B point angle(ANB)as well as an increase in nasion to menton(N-Me),nasion to anterior nasal spine(N-ANS),anterior nasal spine to menton(ANS-Me),with statistical difference(P<0.05).After treatment,AMB-HP and BD-ABM showed an increase(P<0.05),while no statistical difference was found in LA-HP,LA-OP,and OP-HP before and after treatment(P>0.05).An increase was observed in P-OP and L1-NB after treatment(P<0.05).Conclusion Invisible MA appliance can promote mandibular development,correct mandibular retrusion,improve the lateral appearance of the patient,and maintain labial inclination of lower anterior teeth and mandibular plane angle.
