摘要
目的:分析尿毒症血液透析患者发生导管相关性感染(CRBSI)的影响因素。方法:回顾性分析2020年1月至2024年3月该院收治的218例尿毒症血液透析患者的临床资料,统计尿毒症血液透析患者CRBSI的发生情况,采用Logistic回归分析尿毒症血液透析患者发生CRBSI的影响因素。结果:218例尿毒症血液透析患者中,发生CRBS 55例,发生率为25.23%,设为发生组,其余设为未发生组;发生组年龄≥60岁、透析时间≥3年、置管时间≥2周、营养状态较差、透析前1周未使用抗菌药物、转铁蛋白未饱和等占比均高于未发生组,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,透析时间≥3年、置管时间≥2周、透析前1周未使用抗菌药物、转铁蛋白未饱和等均为尿毒症血液透析患者发生CRBSI的危险因素(OR>1,P<0.05)。结论:透析时间≥3年、置管时间≥2周、透析前1周未使用抗菌药物、转铁蛋白未饱和等均为尿毒症血液透析患者发生CRBSI的危险因素。
Objective:To analyze influencing factors of catheter-related infection(CRBSI)in uremic hemodialysis patients.Methods:The clinical data of 218 uremic hemodialysis patients admitted to this hospital from January 2020 to March 2024 were retrospectively analyzed.The incidence of CRBSI in these uremic hemodialysis patients was statistically analyzed.Logistic regression was used to analyze the influencing factors of CRBSI in the uremic hemodialysis patients.Results:There were 55 cases of CRBS in the 218 uremic hemodialysis patients with an incidence of 25.23%,which were set as the occurrence group,and the rest were set as the non-occurrence group.The proportions of the patients with age≥60 years old,dialysis time≥3 years,catheterization time≥2 weeks,poor nutritional status,no use of antibiotics 1 week before dialysis,and transferrin unsaturation in the occurrence group were higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that dialysis time≥3 years,catheterization time≥2 weeks,no use of antibiotics 1 week before dialysis,and transferrin unsaturation were all risk factors for CRBSI in the uremic hemodialysis patients(OR>1,P<0.05).Conclusions:Dialysis time≥3 years,catheterization time≥2 weeks,no use of antibiotics 1 week before dialysis,and transferrin unsaturation are the risk factors for CRBSI in the uremic hemodialysis patients.
