摘要
目的观察在前牙修复时应用微创拔牙即刻种植技术的临床效果。方法选取900例前牙受损需要进行前牙修复的患者,以治疗方式不同分为对照组(450例,微创拔牙后延期种植)和观察组(450例,微创拔牙后即刻种植)。对比两组患者口腔咀嚼能力、种植体边缘骨吸收情况及种植体修复美学效果。结果观察组咬合面、咬合力、咀嚼情况、唇颊外形评分及总分分别为(16.84±1.45)、(18.41±1.69)、(17.54±1.52)、(16.88±1.62)、(69.67±1.88)分,均高于对照组的(15.58±2.11)、(17.88±1.74)、(17.02±1.49)、(16.24±1.60)、(66.72±2.19)分(P<0.05)。术后1、3、6个月,观察组种植体边缘骨吸收量分别为(0.23±0.03)、(0.38±0.02)、(0.43±0.03)mm,均低于对照组的(0.28±0.04)、(0.48±0.03)、(0.50±0.05)mm(P<0.05);术后12个月,观察组种植体边缘骨吸收量略低于对照组,但无统计学差异(P>0.05)。修复完当天、术后3个月、术后6个月、术后12个月,观察组红色美学评分分别为(9.84±1.57)、(10.46±1.62)、(10.75±1.58)、(11.06±1.29)分,对照组分别为(8.87±1.25)、(9.19±1.05)、(9.28±1.14)、(9.36±1.19)分,观察组高于对照组(P<0.05)。结论在前牙修复中采用微创拔牙即刻种植技术可有效提高患者口腔咀嚼能力,减少种植体边缘骨吸收,还可提高患者口腔美观度,减轻患者面容焦虑感。
Objective To observe the clinical effect of minimally invasive extraction and immediate implantation technique in anterior tooth restoration.Methods 900 patients with anterior tooth damage who needed anterior tooth restoration were divided into control group(450 cases,delayed implantation after minimally invasive extraction)and observation group(450 cases,immediate implantation minimally invasive extraction)according to different treatment methods.The oral chewing ability,implant marginal bone resorption,and aesthetic effect of implant restoration were compared between the two groups.Results The scores of occlusal surface,bite force,mastication,lip and buccal appearance and total score of the observation group were(16.84±1.45),(18.41±1.69),(17.54±1.52),(16.88±1.62)and(69.67±1.88)points,which were higher than(15.58±2.11),(17.88±1.74),(17.02±1.49),(16.24±1.60)and(66.72±2.19)points of the control group(P<0.05).At 1,3 and 6 months after surgery,the implant marginal bone resorption in the observation group were(0.23±0.03),(0.38±0.02)and(0.43±0.03)mm,which were lower than(0.28±0.04),(0.48±0.03)and(0.50±0.05)mm in the control group(P<0.05).At 12 months after surgery,the implant marginal bone absorption in the observation group was slightly lower than that in the control group,but there was no statistical difference(P>0.05).On the day after repair,3 months after surgery,6 months after surgery and 12 months after surgery,the red aesthetic scores of the observation group were(9.84±1.57),(10.46±1.62),(10.75±1.58)and(11.06±1.29)points,and those of the control group were(8.87±1.25),(9.19±1.05),(9.28±1.14)and(9.36±1.19)points;the observation group was higher than the control group(P<0.05).Conclusion The use of minimally invasive extraction and immediate implantation technique in anterior tooth restoration can effectively improve the patient's oral chewing ability,reduce the implant marginal bone absorption,improve oral aesthetics,and reduce facial anxiety.
