摘要
目的探讨九江市社区人群新型冠状病毒(新冠病毒)感染者再感染率、免疫水平及影响因素。方法选取2020年1~3月110例原始新冠毒株感染者及2021年1月~2022年9月实验室确诊的991例新冠病毒感染者为再感染调查对象,九江市能仁寺社区和六角石社区的424例新冠病毒感染后康复患者作为抗体水平研究对象。分析新冠病毒感染者康复后抗体阳性的影响因素。结果2020年1~3月110例原始新冠毒株感染者,再感染的有62例,再感染率为56.36%。2021年1月~2022年9月实验室确诊的991例新冠病毒感染者,第1次感染的均为奥密克戎变异株,其中第1次感染时间至调查时间间隔>90 d的仅有39例,再感染的有8例,再感染率为20.51%。424例新冠病毒感染后康复患者中,394例免疫球蛋白G(IgG)抗体阳性,30例Ig G抗体阴性,阳性率为92.92%。IgG抗体阳性与阴性患者的年龄、基础疾病、临床分型、疫苗接种剂次、距最近一次感染时间间隔比较,有明显差异(P<0.05)。424例新冠病毒感染后康复患者中,386例中和抗体阳性,38例中和抗体阴性,阳性率为91.04%。中和抗体阳性与阴性患者的年龄、基础疾病、临床分型、疫苗接种剂次、距最近一次感染时间间隔比较,有明显差异(P<0.05)。Logistic回归分析结果显示,重型+危重型、疫苗接种3剂、距最近一次感染间隔时间<6个月是新冠病毒感染者康复后Ig G抗体和中和抗体阳性的影响因素(P<0.05)。结论重型+危重型、疫苗接种3剂、距最近一次感染间隔时间<6个月是九江市社区人群新冠病毒感染者康复后IgG抗体和中和抗体阳性的影响因素,对于下一阶段疫苗接种策略、防控措施具有一定的指导价值。
Objective To investigate the reinfection rate,immunization level and influencing factors of COVID-19 infected patients in community populations in Jiujiang City.Methods A total of 110 cases of original COVID-19 infection from January to March 2020 and 991 cases of laboratory-confirmed COVID-19 infection from January to September 2022 were selected as the subjects of re-infection investigation,and 424 patients recovered from COVID-19 infection in Nengrensi Community and Jianshi Community of Jiujiang City were selected as the subjects of antibody level study.Factors affecting antibody positivity in patients recovered from COVID-19 infection were analyzed.Results Among the 110 patients infected with the original COVID-19 strain from January to March 2020,62 cases were reinfected,with a reinfection rate of 56.36%.Among the 991 laboratory-confirmed COVID-19 infected patients from January 2021 to September 2022,the first infection was all Omicron variant strains,of which only 39 cases had an interval of>90 d between the time of the first infection and the time of the investigation,and 8 cases were reinfected,with a reinfection rate of 20.51%.Among 424 patients who recovered from COVID-19 infection,394 cases were positive for immunoglobulin(IgG)antibodies and 30 were negative,with a positive rate of 92.92%.There was a significant difference between IgG antibody positive and negative patients in terms of age,underlying disease,clinical typing,number of doses of vaccination,and interval from the last infection(P<0.05).Among 424 patients who recovered from COVID-19 infection,386 cases were positive for neutralizing antibodies and 38 cases were negative,with a positive rate of 91.04%.There were significant differences between neutralizing antibody positive and negative patients in terms of age,underlying disease,clinical typing,number of doses of vaccination,and interval from the last infection(P<0.05).Logistic regression analysis showed that severe+critical type,3 doses of vaccination,and<6 months interval from the last infection were the factors influencing IgG antibody and neutralizing antibody positivity in patients recovery from COVID-19 infection(P<0.05).Conclusion Severe+critical type,3 doses of vaccination,and<6 months interval from the last infection were the factors influencing IgG antibody and neutralizing antibody positivity in patients recovery from COVID-19 infection in Jiujiang City,which have a certain guiding value for the next stage of vaccination strategy,preventive and control measures.
