摘要
目的探讨早产儿出院后追赶生长情况及影响因素。方法选择120例出院早产儿为研究对象,纠正月龄6个月时对其进行随访,记录早产儿出院后追赶生长情况,根据体重有无追赶生长分为A组(出现追赶生长)与B组(无追赶生长),采用多因素Logisitic回归分析追赶生长的影响因素。结果纠正月龄6个月早产儿身长、头围、体重、年龄别身长Z评分(HAZ)、年龄别头围Z评分(HCZ)、年龄别体重Z评分(WAZ)均优于出生时(P<0.05)。纠正月龄6个月后有67例出现体重追赶生长。单因素分析显示:A组孕母年龄<35岁占比83.58%高于B组的60.38%,开始胃肠喂养时间(26.54±4.67)h早于B组的(34.25±5.21)h,早期喂养不耐受占比17.91%低于B组的37.74%(P<0.05)。多因素Logistic回归分析显示:孕母年龄≥35岁、开始胃肠喂养时间≥30 h是纠正月龄6个月早产儿追赶生长的影响因素(P<0.05)。结论纠正月龄6个月早产儿存在追赶生长情况,孕母年龄、开始胃肠喂养时间是其影响因素。
Objective To explore the catch-up growth and influencing factors in premature infants discharged.Methods 120 discharged preterm infants were selected as the study subjects,and were followed up a corrected age of 6 months and their catch-up growth was recorded after discharge.The premature infants were divided into Group A(with catch-up growth)and Group B(without catch-up growth)based on their weight.Multivariate Logistic regression was used to analyze the factors affecting their catch-up growth.Results Height,head circumference,weight,height for age Z-score(HAZ),head circumference for age Z-score(HCZ),and weight for age Z-score(WAZ)of premature infants after a corrected age of 6 months were better than those at birth(P<0.05).67 cases showed weight catch-up growth after a corrected age of 6 months.Univariate analysis showed that 83.58%of pregnant women in Group A were<35 years of age,which was higher than 60.38%in Group B;the time to start of gastrointestinal feeding was(26.54±4.67)h,which was earlier than(34.25±5.21)h in Group B;the proportion of early feeding intolerance was 17.91%,which was lower than 37.74%in Group B(P<0.05).Multivariate Logistic regression analysis showed that maternal age≥35 years and the time to start of gastrointestinal feeding≥30 h were risk factors affecting catch-up growth of premature infants after a corrected age of 6 months(P<0.05).Conclusion The catch-up growth of premature infants after a corrected age of 6 months is affected by maternal age and the time to start of gastrointestinal feeding.
