摘要
目的分析小剂量奥氮平联合舍曲林治疗老年脑卒中后抑郁(PSD)患者的临床疗效。方法选取80例老年脑卒中后抑郁患者,根据双模拟法分成参照组和研究组,每组40例。参照组单用小剂量奥氮平治疗,研究组应用小剂量奥氮平联合舍曲林治疗。对比两组疗效、汉密尔顿抑郁量表(HAMD)评分、用药不良反应率及生活质量评分。结果研究组总有效率97.50%较参照组的80.00%高,差异有统计学意义(P<0.05)。研究组治疗2、4、6周HAMD评分分别为(19.24±3.08)、(13.15±5.22)、(7.12±5.08)分,均显著低于参照组的(22.15±6.27)、(16.48±6.31)、(10.43±4.52)分,差异有统计学意义(P<0.05)。对比参照组(25.00%),研究组不良反应发生率(7.50%)更低,差异有统计学意义(P<0.05)。治疗前,两组生理功能、躯体疼痛、社会功能、心理功能评分对比没有差异(P>0.05);治疗后,两组生理功能、躯体疼痛、社会功能、心理功能评分均高于治疗前,且研究组均高于参照组,差异有统计学意义(P<0.05)。结论老年脑卒中后抑郁患者应用小剂量奥氮平联合舍曲林治疗,可提高临床疗效和安全性,临床推广价值更高。
Objective To analyze the clinical efficacy of low-dose olanzapine combined with sertraline in the treatment of senile patients with post-stroke depression(PSD).Methods A total of 80 elderly patients with post-stroke depression were selected and divided into reference group and study group according to double-dummy method,with 40 cases in each group.The reference group was treated with low-dose olanzapine alone,and the study group was treated with low-dose olanzapine combined with sertraline.The efficacy,Hamilton Depression Scale(HAMD)score,adverse drug reaction rate and quality of life score were compared between the two groups.Results The total effective rate of 97.50%in the study group was higher than 80.00%in the reference group,and the difference was statistically significant(P<0.05).HAMD scores of the study group at 2,4 and 6 weeks of treatment were(19.24±3.08),(13.15±5.22)and(7.12±5.08)points,which were significantly lower than(22.15±6.27),(16.48±6.31)and(10.43±4.52)points of the reference group,and the difference was statistically significant(P<0.05).Compared with the reference group(25.00%),the incidence of adverse reactions in the study group(7.50%)was lower,and the difference was statistically significant(P<0.05).Before treatment,there were no differences in the scores of physical function,bodily pain,social function and psychological function between the two groups(P>0.05).After treatment,the scores of physical function,bodily pain,social function and psychological function in both groups were higher than those before treatment,and the study group was higher than the reference group.The difference was statistically significant(P<0.05).Conclusion Low-dose olanzapine combined with sertraline can improve the clinical efficacy and safety of elderly patients with post-stroke depression,and has higher clinical promotion value.
