摘要
目的探寻细菌鉴定联合药敏分析在尿路感染(UTI)诊断中的价值。方法选择41例疑似UTI患者,分别进行常规尿液检验、细菌鉴定联合药敏分析及尿流式细胞术联合基质辅助激光解析电离飞行时间质谱(MALDI-TOF MS)。分析两种检验方式的检验结果;以尿流式细胞术联合MALDITOF MS结果为金标准,比较两种检验方式的诊断效能;分析病原菌分布情况。结果尿流式细胞术联合MALDI-TOF MS检验结果显示,阳性32例,阴性9例;常规尿液检验结果显示阳性31例,阴性10例;细菌鉴定联合药敏分析结果显示阳性32例,阴性9例。以尿流式细胞术联合MALDI-TOF MS结果为金标准,细菌鉴定联合药敏分析的诊断敏感度、特异度、准确性分别为96.88%(31/32)、88.89%(8/9)、95.12%(39/41),明显高于常规尿液检验的81.25%(26/32)、44.44%(4/9)、73.17%(30/41)(P<0.05)。尿流式细胞术联合MALDI-TOF MS共检出53株病原菌,以大肠埃希菌和奇异变形杆菌为主。细菌鉴定联合药敏分析中,大肠埃希菌、奇异变形杆菌存在未检出情况,其他病原菌的检出率均为100.00%。结论将细菌鉴定联合药敏分析方式应用在UTI的诊断中,能够使患者诊断准确性提升,还可明确鉴别其病原菌类型,可综合判断UTI病情,具有较高的诊断价值。
Objective To explore the value of bacterial identification combined with drug susceptibility analysis for urinary tract infections(UTI).Methods 41 patients with suspected UTI were selected,and all underwent routine urine testing,bacterial identification combined with drug susceptibility analysis and urinary flow cytometry combined with matric-assisted laser desorption/ionization mass spectrometry(MALDT-TOF MS).The results of the two tests were analyzed,and the diagnostic efficiency of the two tests was compared with the results of urinary flow cytometry combined with MALDT-TOF MS as the gold standard;the distribution of pathogenic bacteria was analyzed.Results Urinary flow cytometry combined with MALDT-TOF MS results showed 32 positive cases and 9 negative cases;routine urine testing was positive in 31 cases and negative in 10 cases;bacterial identification combined with drug susceptibility analysis was positive in 32 cases and negative in 9 cases.Using the urinary flow cytometry combined with MALDT-TOF MS results as the gold standard,the diagnostic sensitivity,specificity and accuracy of bacterial identification combined with drug susceptibility analysis were 96.88%(31/32),88.89%(8/9)and 97.56%(40/41),which were significantly higher than 81.25%(26/32),44.44%(4/9)and 73.17%(30/41)of the routine urine tests(P<0.05).A total of 53 strains of pathogenic bacteria were detected in urinary flow cytometry combined with MALDT-TOF MS,with Escherichia coli and Proteus mirabilis predominating.In bacterial identification combined with drug susceptibility analysis,there was non-detection of Escherichia coli and Proteus mirabilis,while the detection rate of other pathogenic bacteria was 100.00%.Conclusion The application of bacterial identification combined with drug susceptibility analysis in the diagnosis of UTI can improve the diagnostic accuracy of patients,clearly identify the type of pathogen,and comprehensively judge the condition of UTI,which has high diagnostic value.
