摘要
目的分析脊柱损伤法医学鉴定中X线、CT检查的应用价值。方法60例疑似脊柱损伤患者,均接受X线、CT检查。比较X线、CT单独检查及联合检查的诊断效能,X线、CT单独检查及联合检查的脊柱损伤检出率。结果金标准结果显示,阳性50例、阴性10例。X线、CT联合检查的灵敏性98.00%(49/50)、准确性96.67%(58/60)、阴性预测值90.00%(9/10)均高于X线检查的72.00%(36/50)、70.00%(42/60)、30.00%(6/20)及CT检查的80.00%(40/50)、80.00%(48/60)、44.44%(8/18)(P<0.05);X线、CT单独检查与联合检查的特异性、阳性预测值比较差异不显著(P>0.05)。X线、CT联合检查对骨折伴移位检出率100.00%高于X线检查的45.45%、CT检查的54.55%(P<0.05);X线、CT联合检查对压缩性骨折、粉碎性骨折检出率高于X线、CT单独检查,但差异不显著(P>0.05)。结论法医学鉴定脊柱损伤中X线、CT联合检查的诊断价值较单独检查高。
Objective To analyze the application value of X-ray and CT examinations in forensic identification of spinal injury.Methods All 60 patients with suspected spinal injury received X-ray and CT examinations.The diagnostic efficiency and detection rate of spinal injury of X-ray,CT alone and their combination were compared.Results The gold standard results showed 50 positive cases and 10 negative cases.The sensitivity of combined X-ray and CT examination was 98.00%(49/50),the accuracy was 96.67%(58/60)and the negative predictive value was 90.00%(9/10),which were higher than 72.00%(36/50),70.00%(42/60)and 30.00%(6/20)of X-ray alone,and 80.00%(40/50),80.00%(48/60)and 44.44%(8/18)of CT alone(P<0.05).There were no significant differences in the specificity and positive predictive value among X-ray,CT alone and their combination(P>0.05).The detection rate of fracture with displacement by combined X-ray and CT examination was 100.00%,which was higher than 45.45%by X-ray and 54.55%by CT(P<0.05).The detection rates of compression fracture and comminuted fracture by combined X-ray and CT examination were higher than those of X-ray and CT alone,but the difference was not significant(P>0.05).Conclusion Combined X-ray and CT examination shows superior diagnostic value compared to X-ray or CT alone in forensic identification of spinal injury.
