摘要
目的构建急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后个案管理模式。方法采用课题研究型品管圈方法,从医院、社区卫生服务中心及患者家庭3方面,分析AMI患者PCI术后个案管理现状与问题,借鉴奥马哈系统框架进行针对性改进。结果奥马哈系统个案管理质量达标率由40%提升至98%;AMI患者PCI术后出院准备度达标率由52.0%提升至90.5%,随访计划完成率由52.5%提升至95.8%,就医满意度由82.5%提升至98.0%,3个月内非计划再住院率由10%降低至4%,出院后6个月服药依从性达标率由62.0%提升至91.5%。结论基于奥马哈系统构建的AMI患者PCI术后个案管理模式,为患者提供了个性化护理方案,有效地提高了患者的健康管理能力,减少了再住院率,提高了生存质量。
Objective To develop a case management model for patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods Utilizing a research-oriented quality control circle approach,the current status and issues of case management for AMI patients post-PCI were analyzed from three perspectives:the hospital,community health service center,and patients'family.Targeted improvements were made by drawing on the framework of the Omaha System.Results The compliance rate for the quality of case management based on the Omaha System increased from 40%to 98%.The readiness for discharge rate among AMI patients post-PCI improved from 52.0%to 90.5%,the completion rate of follow-up plans increased from 52.5%to 95.8%,patient satisfaction with the healthcare experience increased from 82.5%to 98.0%,the unplanned readmission rate within three months decreased from 10%to 4%,and the medication adherence rate six months post-discharge improved from 62.0%to 91.5%.Conclusion The case management model for AMI patients post-PCI,developed based on the Omaha System,provides personalized care plans,effectively enhances patients'health management capabilities,reduces readmission rates,and improves quality of life.
