摘要
名老中医李发枝教授辨证论治痛风,在减轻患者临床疼痛症状、平稳降低尿酸水平、减少痛风复发等方面疗效显著,本文系统介绍李发枝教授辨证治疗痛风的临床经验。李发枝教授提出“湿邪”是痛风的关键致病因素,且湿多“夹热”“夹寒”“夹郁”“夹虚”,贯穿于痛风发病的急性期与慢性期,急性痛风辨为湿热留恋型,慢性期痛风辨为寒湿蕴结型、气郁湿侵型、气虚湿阻型。总体治疗以清热利湿、散寒除湿、理气祛湿、健脾化湿为辨证治则,选用经典方剂当归拈痛汤、甘草附子汤、柴苓汤、防己黄芪汤为主,临床取得满意疗效。
Professor Li Fazhi,a renowned traditional Chinese medicine(TCM)physician,has achieved remarkable clinical outcomes in alleviating pain symptoms,stabilizing uric acid levels,and reducing gout recurrence through syndrome differentiation and treatment.This article systematically introduces Professor Li Fazhi clinical experience in treating gout.He emphasizes that dampness is the key pathogenic factor in gout,often accompanied by heat,cold,stagnation,or deficiency,which are present in both the acute and chronic phases of the disease.Acute gout is categorized as the damp-heat retention type,while chronic gout is divided into the cold-damp accumulation type,Qi stagnation with damp invasion type,and Qi deficiency with damp obstruction type.The overall treatment principles include clearing heat and resolving dampness,dispersing cold and eliminating dampness,regulating Qi and removing dampness,and strengthening the spleen to transform dampness.Classical prescriptions such as Chinese Angelica Pain-Alleviating Decoction,Licorice and Aconite Accessory Root Decoction,Chai-Ling Decoction,and Stephania Root and Astragalus Decoction are primarily used,achieving satisfactory clinical efficacy.
