摘要
目的:探讨雷火灸联合负压封闭引流治疗Ⅲ~Ⅳ期压疮患者临床疗效、及对肉芽组织生长因子的影响。方法:采用前瞻性随机对照设计,选取2024年3月~2025年3月本院收治的Ⅲ~Ⅳ期压疮患者40例,分为对照组和观察组各20例。对照组采用外科换药干预,观察组采用雷火灸联合负压封闭引流。比较两组患者治疗前后压力性损伤愈合评分(PUSH)、中医症候积分、VAS评分、肉芽组织出现时间、创面愈合时间、肉芽组织生长因子(EGF、bFGF、VEGF)水平。结果:治疗后,两组患者的PUSH(组织类型、24 h渗出量、创面面积、总分)评分、中医症候积分、VAS评分均较治疗前降低(P<0.05),EGF、bFGF、VEGF水平均较治疗前升高(P<0.05),且观察组PUSH(组织类型、24 h渗出量、创面面积、总分)评分、中医症候积分、VAS评分均低于对照组(P<0.05),肉芽组织出现时间、创面愈合时间均短于对照组(P<0.05),EGF、bFGF、VEGF水平显著高于对照组(P<0.05)。结论:对Ⅲ~Ⅳ期压疮患者采用VSD联合雷火灸治疗,可有效缩小损伤面积、减少渗液,促进健康肉芽组织生成,减轻疼痛,并通过调节EGF、bFGF、VEGF等关键细胞因子的表达水平,加速创面修复进程。
Objective:To investigate the clinical efficacy of thunder fire moxibustion combined with negative pressure closed drainage in the treatment of patients with stage Ⅲ~Ⅳ pressure ulcers,and its effect on the growth factors of granulation tissue.Methods:This study adopted a prospective randomized controlled design,selecting patients with stage Ⅲ~Ⅳ pressure ulcers admitted to our hospital from March 2024 to March 2025 as the research subjects.A total of 40 patients were included,and they were evenly divided into a control group and an observation group using a random number table method,with 20 patients in each group.The control group was intervened with surgical dressing change,and the observation group was treated with thunder fire moxibustion combined with negative pressure closed drainage.Compare the pressure ulcer scale for healing(PUSH),TCM syndrome score,VAS score,appearance and healing time of granulation tissue,and granulation tissue growth factor(EGF,bFGF,VEGF)levels between two groups of patients before and after treatment.Results:After treatment,the PUSH(tissue type,24-hour exudate,wound area,total score)score,TCM symptom score,and VAS score of both groups of patients decreased compared to before treatment(P<0.05),while the levels of EGF,bFGF,and VEGF increased compared to before treatment(P<0.05).Moreover,the PUSH(tissue type,24-hour exudate,wound area,total score)score,TCM symptom score,and VAS score of the observation group were lower than those of the control group(P<0.05),and the appearance time of granulation tissue and wound healing time were shorter than those of the control group(P<0.05).The levels of EGF,bFGF,and VEGF were significantly higher than those of the control group(P<0.05).Conclusion:The combination of VSD and thunder fire moxibustion treatment for stage III~IV pressure ulcers can effectively reduce the area of injury,decrease exudate,promote healthy granulation tissue generation,alleviate pain,and accelerate wound repair process by regulating the expression levels of key cytokines such as EGF,bFGF,VEGF,etc.
