摘要
目的探讨心理弹性与疾病感知在中青年炎症性肠病患者领悟社会支持与恐惧疾病进展间的链式中介效应,为缓解该人群恐惧疾病进展水平提供理论依据。方法研究对象选取广州市内2所三级甲等医院2024年1-7月就诊的中青年炎症性肠病患者。使用一般资料调查问卷、恐惧疾病进展简化量表、心理弹性量表简版、简易版疾病感知问卷、领悟社会支持量表对中青年炎症性肠病患者开展调查。采用SPSS及Bootstrap法构建链式中介模型并检验。结果中青年炎症性肠病患者恐惧疾病进展与心理弹性、疾病感知、领悟社会支持两两显著相关(P<0.05)。中介效应分析显示,心理弹性、疾病感知在领悟社会支持与恐惧疾病进展间链式中介作用显著,总间接效应为-0.346。心理弹性占总间接效应的33.24%,疾病感知占总间接效应的41.91%,心理弹性与疾病感知占总中介效应的24.85%。结论领悟社会支持是恐惧疾病进展的重要影响因素,心理弹性和疾病感知在其中起链式中介作用,未来建议加强多维度的社会支持系统,以帮助患者增强心理弹性,纠正不良的疾病感知,从而降低恐惧疾病进展的水平。
Objective To explore the chain mediating role of psychological resilience and illness perception in the relationship between perceived social support and fear of disease progression among young and middle-aged patients with inflammatory bowel disease(IBD),and to provide a theoretical basis for reducing fear of disease progression in this population.MethodsA cross-sectional survey was conducted between January and July 2024 among young and middle-aged IBD patients from two tertiary hospitals in Guangzhou.Data were collected using a general demographic questionnaire,the Fear of Progression Questionnaire-Short Form(FoP-Q-SF),Brief Resilience Scale(BRS),Brief Ilness Perception Questionnaire(BIPQ),and the Perceived Social Support Scale(PSSS).SPSS and the Bootstrap method were used to construct and test a chain mediation model.Results Fear of disease progression was significantly correlated with psychological resilience,illness perception,and perceived social support(P<0.05).Mediation analysis revealed that psychological resilience and illness perception played a significant chain mediating role between perceived social support and fear of disease progression.The total indirect effect was-0.346,with psychological resilience accounting for 33.24%,illness perception for 41.91%,and the combined chain mediation accounting for 24.85%of the total indirect effect.Conclusion Perceived social support significantly influences fear of disease progression in young and middle-aged IBD patients.Psychological resilience and illness perception serve as key chain mediators in this relationship.Interventions should focus on enhancing multi-dimensional social support systems to improve resilience and correct maladaptive illness perceptions,thereby alleviating fear of disease progression.
