雷火灸联合上肢康复机器人对脑卒中后偏瘫性肩痛患者肩关节疼痛、上肢功能及肌骨超声指标的影响 被引量:1

摘要 目的 探究雷火灸联合上肢康复机器人对脑卒中后偏瘫性肩痛(HSP)患者肩关节疼痛、上肢功能及肌骨超声指标的影响,为有效地减轻患者肩关节疼痛,改善上肢功能提供新的治疗方案。方法 前瞻性选取2022年3月至2024年3月浙江中医药大学附属第三医院收治的脑卒中后HSP患者120例,随机数字表法分为雷火灸组(40例)、康复机器人组(40例)和联合组(40例)。三组均进行常规治疗和护理,在此基础上,雷火灸组采用雷火灸治疗,康复机器人组采用上肢康复机器人干预,联合组采用雷火灸和上肢康复机器人联合治疗。治疗前后,采用疼痛数字表法(NRS)评估肩关节疼痛,采用上肢Fugl-Meyer运动功能评定量表(FMA)评估上肢功能,采用肌骨超声测量肩峰至大结节间距,采用Barthel指数量表(BI)评估患者日常生活生活能力。结果 治疗前,三组患者的NRS、FMA、BI评分及肩峰至大结节间距差异无统计学意义(P>0.05),治疗后,联合组的NRS评分、肩峰至大结节间距低于雷火灸组和康复机器人组(P<0.05),FMA及BI评分显著高于雷火灸组和康复机器人组(均P<0.05)。结论 雷火灸联合上肢康复机器人应用于脑卒中后HSP患者,可有效缓解肩关节疼痛,改善上肢功能,增强肩周肌肉收缩水平。 Objective To investigate the effects of Thunder Fire Moxibustion combined with an upper limb rehabilitation robot on shoulder pain,upper limb function,and musculoskeletal ultrasound indicators in patients with post-stroke hemiplegic shoulder pain(HSP),and to provide a novel therapeutic approach for relieving shoulder pain and improving upper limb function.Methods A total of 120 patients with HSP after stroke,treated at the Third Affiliated Hospital of Zhejiang University of Traditional Chinese Medicine from March 2022 to March 2024,were prospectively selected and randomly divided into three groups using a random number table:Thunder Fire Moxibustion Group(n=40),Rehabilitation Robot Group(n=40),and Combination Group(n=40).All groups received conventional treatment and nursing care.On this basis,the Thunder Fire Moxibustion Group received Thunder Fire Moxibustion treatment alone;the Rehabilitation Robot Group received upper limb rehabilitation robot intervention alone;and the Combination Group received both interventions simultaneously.The Numerical Rating Scale(NRS)was used to assess shoulder pain,the Fugl-Meyer Assessment(FMA)to evaluate upper limb motor function,musculoskeletal ultrasound to measure the acromion-to-greater tuberosity distance,and the Barthel Index(BI)to assess activities of daily living,before and after treatment.Results Before treatment,there were no significant differences in NRS,FMA,BI scores,or acromion-to-greater tuberosity distance among the three groups(P>0.05).After treatment,the Combination Group had significantly lower NRS scores(P<0.05)and acromion-to-greater tuberosity distance(P<0.05)compared with the Thunder Fire Moxibustion and Robot Groups.Additionally,the Combination Group had significantly higher FMA scores(P<O.05)and BI scores(P<0.05)than the other two groups.Conclusion The combination of Thunder Fire Moxibustion and upper limb rehabilitation robot effectively relieves shoulder pain,improves upper limb function,and enhances shoulder muscle contraction ability in patients with post-stroke HSP.
