摘要
目的探讨左心室舒张末容积校正的QRS持续时间(QRSd/LVEDV)、完全性左束支传导阻滞(CLBBB)、血清胱抑素C(Cys-C)水平与慢性心力衰竭患者心脏再同步化治疗(CRT)应答的相关性分析。方法收集2022年3月至2023年3月福建医科大学附属泉州第一医院收治的接受CRT治疗的慢性心力衰竭患者120例,随访6个月,根据患者是否应答将患者分为无应答组35例和应答组85例。比较两组患者临床资料,采用多因素logistic回归分析影响CRT应答反应的相关因素,并比较不同因素对CRT应答的预测价值。结果两组患者病因、CLBBB、QRSd/LVEDV、Cys-C水平比较差异具有统计学意义(P<0.05)。经过logistic回归分析,CLBBB(OR=1.581)、QRSd/LVEDV(OR=2.392)、Cys-C(OR=0.453)是慢性心力衰竭患者CRT应答的影响因素(P<0.05)。CLBBB、QRSd/LVEDV及Cys-C对慢性心力衰竭患者CRT治疗应答预测价值的AUC分别为0.649、0.729、0.868。结论QRSd/LVEDV、CLBBB、Cys-C是心力衰竭患者CRT术后应答的独立预测因子和影响因素,为CRT治疗提供有价值的信息。
Objective To investigate the correlation between left ventricular end-diastolic volume adjusted QRS duration(QRSd/LVEDV),complete left bundle branch block(CLBBB),serum cystatin C(Cys-C)level and cardiac resynchronization therapy(CRT)response in patients with chronic heart failure(CHF).Methods A total of 120 patients with CHF who received CRT treatment in Quanzhou First Hospital Affiliated to Fujian Medical University from March 2022 to March 2023 were collected and followed up for 6 months.According to whether the patients responded,they were divided into a no-response group(35 cases)and a response group(85 cases).The clinical data of the two groups were compared,and the relevant factors affecting CRT response were analyzed by multivariate logistic regression,and the prognostic value of different factors on CRT response was compared.Results There were significant differences in the etiology,CLBBB,QRSd/LVEDV and Cys-C levels between the two groups(P<0.05).Logistic regression analysis showed that CLBBB(OR=1.581),QRSd/LVEDV(OR=2.392),and Cys-C(OR=0.453)were the influencing factors leading to the response of patients(P<0.05).The AUC values of CLBBB,QRSd/LVEDV and Cys-C for predicting CRT response in patients with CHF were 0.649,0.729 and 0.868,respectively.Conclusion QRSd/LVEDV,CLBBB and Cys-C are independent predictors and influencing factors of postoperative response to CRT in patients with CHF,providing valuable information for CRT treatment.
