磁共振扩散张量成像评估2型糖尿病患者脑白质微结构改变及与认知功能的相关性

Evaluation on Brain White Matter Microstructure Changes in Type 2 Diabetes Patients by Diffusion Tensor Imaging and the Correlation with Cognitive Function

摘要 目的探讨磁共振扩散张量成像(DTI)评估2型糖尿病(T2DM)患者的脑白质微结构改变及与认知功能的相关性。方法选取90例T2DM患者作为病例组,90例健康受试者作为对照组。两组均进行DTI检查,比较两组的不同脑区脑白质FA值、MD值以及MoCA评分,分析MoCA评分与FA值、MD值的相关性。结果病例组各脑区的脑白质FA值均低于对照组,MD值均高于对照组(P<0.05)。病例组的视觉空间与执行、语言、延迟回忆、定向维度评分以及MoCA总分均低于对照组(P<0.05)。Pearson相关性分析显示,T2DM患者不同脑区的脑白质FA值与MoCA总分均呈显著正相关(r>0,P<0.05),MD值与MoCA总分均呈显著负相关(r<0,P<0.05)。结论DTI显示T2DM患者的脑白质微结构明显改变,各脑区的FA值降低、MD值升高,脑白质微结构改变与认知功能密切相关。 Objective To explore the brain white matter microstructure changes in patients with type 2 diabetes mellitus(T2DM)evaluated by diffusion tensor imaging(DTI)and the correlation with cognitive function.Methods 90 T2DM patients were selected as the case group,and 90 healthy subjects were selected as the control group.Both groups underwent DTI examination,the FA and MD values of brain white matter in different brain regions,and the MoCA scores of the two groups were compared,and the correlations of MoCA score with FA and MD values were analyzed.Results The FA values of brain white matter in each brain region of the case group were lower than those of the control group,and the MD values were higher than those of the control group(P<0.05).The scores of visual space and execution,language,delayed recall,and orientation dimensions,and the total MoCA score of the case group were lower than those of the control group(P<0.05).Pearson correlation analysis showed that the brain white matter FA values in different brain regions of T2DM patients were significantly positively correlated with the total MoCA score(r>0,P<0.05),while the MD values were significantly negatively correlated with the total MoCA score(r<0,P<0.05).Conclusions DTI shows significant changes in the brain white matter microstructure in T2DM patients,with decreased FA values and increased MD values in various brain regions.The changes in brain white matter microstructure are closely related to cognitive function.
