摘要
目的 探究血清微小RNA-136-5p(miR-136-5p)、微小RNA-377-3p(miR-377-3p)表达对子宫内膜癌(EC)患者早期诊断及预后的临床价值。方法 选择山西医科大学第一医院2019年2月至2021年2月收治的EC患者283例作为疾病组,同期收治的子宫良性病变患者283例作为子宫良性病变组,另选取同期检查的子宫内膜正常者283例作为对照组。采用qRT-PCR法检测血清miR-136-5p、miR-377-3p表达水平;运用ROC曲线分析血清miR-136-5p、miR-377-3p对早期EC患者的诊断价值;分析EC患者血清miR-136-5p、miR-377-3p表达水平与临床病理特征的关系;采用Kaplan-Meier法分析血清miR-136-5p和miR-377-3p表达与EC患者预后的关系。结果 疾病组血清miR-136-5p、miR-377-3p表达水平明显低于子宫良性病变组、对照组,子宫良性病变组血清miR-136-5p、miR-377-3p表达水平明显低于对照组(P<0.05)。血清miR-136-5p、miR-377-3p及二者联合诊断EC的曲线下面积(AUC)分别为0.791、0.758、0.840,优于各自单独诊断(Z_(联合-miR-136-5P)=2.768、Z_(联合-miR-377-3P)=4.120,P=0.006、P<0.001)。FIGO分期为Ⅲ~Ⅳ期、肌层浸润深度≥1/2、低分化程度和发生淋巴结转移的EC患者血清miR-136-5p、miR-377-3p低表达占比明显高于FIGO分期为Ⅰ~Ⅱ期、肌层浸润深度<1/2、中高分化程度和未发生淋巴结转移的EC患者(P<0.05)。血清miR-136-5p高表达EC患者3年生存率(78.32%)高于miR-136-5p低表达患者(35.71%)(χ^(2)=72.053,P<0.001);血清miR-377-3p高表达EC患者3年生存率(81.69%)高于miR-377-3p低表达患者(32.62%)(χ^(2)=89.934,P<0.001)。结论 EC患者血清miR-136-5p、miR-377-3p表达水平明显降低,二者联合对EC具有更高的诊断价值,对预后评估有一定的临床应用价值。
Objectivee To investigate the clinical value of serum microRNA-136-5p(miR-136-5p)and microRNA-377-3p(miR-377-3p)expression in the early diagnosis and prognosis of endometrial cancer(EC)patients.Methods A total of 283 EC patients admitted to our hospital from February 2019 to February 2021 were selected as the disease group,283 patients with benign uterine lesions were included as benign uterine lesion group,and 283 patients with normal endometrium were selected as the control group.qRT-PCR was used to detect serum miR-136-5p and miR-377-3p expression levels.ROC curves were used to analyze the diagnostic value of serum miR-136-5p and miR-377-3p in patients with early EC.The relationship between serum miR-136-5p and miR-377-3p expression levels with clinicopathological features in EC patients was analyzed.Kaplan-Meier method was used to analyze the relationship between serum miR-136-5p and miR-377-3p expression and the prognosis of EC patients.Results The serum expression levels of miR-136-5p and miR-377-3p in the disease group were greatly lower than those in the benign uterine lesion group and the control group,and the serum expression levels of miR-136-5p and miR-377-3p in the benign uterine lesion group were greatly lower than those in the control group(P<0.05).The area under the curve(AUC)of serum miR-136-5p,miR-377-3p and their combination for EC was 0.791,0.758 and 0.840,respectively,the combined diagnosis was better than individual diagnoses(Z.combinio miR136-sp=2.768 and Z omimaiomr 37-ap=4.120,P=0.006 and P<0.001).The proportions of low serum expression of miR-136-5p and miR-377-3p in EC patients with FIGO stages II~IN,muscle invasion depth≥1/2,low differentiation and lymph node metastasis were greatly higher than those in EC patients with FIGO stages I~II,muscle invasion depth<1/2,medium to high differentiation and no lymph node metastasis(P<0.05).The 3-year survival rate of EC patients with high serum miR-136-5p expression(78.32%)was higher than that of patients with low expression of miR-136-5p(35.71%)(x^(2)=72.053,P<0.001).The 3-year survival rate of EC patients with high serum miR-377-3p expression(81.69%)was higher than that of patients with low expression of miR-377-3p(32.62%)(x^(2)=89.934,P<0.001).Conclusion The expression levels of miR-136-5p and miR-377-3p in the serum of EC patients are greatly reduced.The combination of the two has higher diagnostic value for EC and certain clinical application value for prognosis evaluation.
