摘要
目的 探讨血清细胞骨架相关蛋白4(CKAP4)、内皮细胞特异分子1(ESM-1)水平与非小细胞肺癌(NSCLC)患者临床病理特征的关系及其在预后中的价值。方法 回顾性收集2020年1月至2022年1月在河南大学第一附属医院就诊的110例NSCLC患者(NSCLC组)临床资料,随访2年后根据癌因死亡情况分为生存组85例和死亡组25例;同期选取本院健康体检人群110例为对照组。采用ELISA法检测血清CKAP4、ESM-1水平;采用ROC曲线分析血清CKAP4、ESM-1水平评估NSCLC患者预后的价值,以CKAP4、ESM-1截断值为界将患者进行高低表达分组,分析与临床病理特征相关性;采用多因素logistic回归分析NSCLC患者血清CKAP4、ESM-1水平的影响因素。结果 NSCLC组血清CKAP4、ESM-1水平高于对照组(P<0.01)。死亡组血清CKAP4、ESM-1水平高于生存组(P<0.01)。血清CKAP4、ESM-1水平单独评估患者死亡的AUC为0.840、0.808,低于联合的AUC为0.924(P<0.05)。CKAP4高表达组TNM分期Ⅲ~Ⅳ比例、有淋巴结转移比例高于CKAP4低表达组(P<0.01);ESM-1高表达组低分化比例、TNM分期Ⅲ~Ⅳ比例、有淋巴结转移比例高于ESM-1低表达组(P<0.01)。TNM分期Ⅲ~Ⅳ、有淋巴结转移是CKAP4高表达的独立危险因素(P<0.01);低分化、TNM分期Ⅲ~Ⅳ、有淋巴结转移是ESM-1高表达的独立危险因素(P<0.01)。结论 血清CKAP4、ESM-1水平可有效预测NSCLC患者预后,二者与临床病理特征密切相关,其中TNM分期Ⅲ~Ⅳ、有淋巴结转移影响CKAP4高表达,低分化、TNM分期Ⅲ~Ⅳ、有淋巴结转移影响ESM-1高表达。
Objective To investigate the relationship between serum cytoskeleton-associated protein 4(CKAP4)and endothelial cell-specific molecule 1(ESM-1)levels and clinicopathological features of non-small cell lung cancer(NSCLC)patients and their value in prognosis.Methods The clinical data of 11o NSCLC patients(NSCLC group)who were admitted to our hospital from January 2020 to January 2022 were retrospectively collected,and after 2 years of follow-up,they were separated into survival group(85 cases)and death group(25 cases)according to the death of cancer.During the same period,llo patients were selected as the control group.ELISA was used to detect serum CKAP4 and ESM-1 levels.ROC curves were used to analyze the value of serum CKAP4 and ESM-1 levels in assessing the prognosis of NSCLC patients.The cut-off values of CKAP4 and ESM-1 were used as the boundaries to group the high and low expression of the patients,and their correlation with clinicopathological characteristics was analyzed.Multivariate logistic regression analysis was used to analyze the influencing factors of serum CKAP4 and ESM-1 levels in NSCLC patients.Results The serum levels of CKAP4 and ESM-1 in the NSCLC group were higher than those in the control group(P<0.O1).The serum levels of CKAP4 and ESM-1 in the death group were higher than those in the survival group(P<0.01).The AUC of serum CKAP4 and ESM-1 levels alone in the assessment of mortality was 0.840 and 0.808,which was lower than that of the combined AUC of 0.924(P<0.05).The proportions of TNM stage II~IV and lymph node metastasis in the CKAP4 high expression group were higher than those in the low CKAP4 expression group(P<0.O1).The proportion of low differentiation,TNM stage II~IV,and lymph node metastasis in the ESM-1 high expression group were higher than those in the ESM-1 low expression group(P<0.01).TNM stage II~IV and lymph node metastasis were independent risk factors for high expression of CKAP4(P<0.01).Low differentiation,TNM stage II~IV,and lymph node metastasis were independent risk factors for high expression of ESM-1(P<0.01).Conclusion Serum levels of CKAP4 and ESM-1 can effectively predict the prognosis of NSCLC patients,and they are closely related to clinicopathological features.TNM stage II~IV and lymph node metastasis affect the high expression of CKAP4,and the low differentiation,TNM stage II~IV,and lymph node metastasis affect the high expression of ESM-1.
