动态监测PCT、IL-6水平预测ICU肿瘤并发感染患者预后的价值

摘要 目的 动态监测重症加强护理病房(ICU)肿瘤患者并发感染后降钙素原(PCT)和白细胞介素-6(IL-6)水平,评估PCT和IL-6对ICU肿瘤并发感染患者预后的预测价值。方法 选取2022年7月至2023年12月入住ICU的208例肿瘤并发感染患者作为研究对象,根据入住ICU期间生存预后分为存活组163例和死亡组45例。所有患者入住ICU第1、3、5、7天采集血液标本检测血清PCT和血浆IL-6,比较两组间两个指标水平的差异,并绘制ROC曲线评估两个指标单一及联合检测对肿瘤并发感染患者预后的预测价值。结果 死亡组中机械通气和发生休克的患者占比均明显高于存活组,差异均具有统计学意义(P<0.05);死亡组与存活组相比,第7天PCT、IL-6水平差异均有统计学意义(P<0.05);通过ROC曲线分析,第7天PCT、IL-6及联合对肿瘤并发感染患者存活和死亡具有较好的预测效能,差异均具有统计学意义(P<0.05),预测死亡的AUC面积分别为0.679、0.664和0.699,灵敏度分别为75.6%、60%和77.8%,特异度分别为61.3%、63%和60.7%。结论 动态监测PCT、IL-6水平对ICU肿瘤并发感染患者生存预后有一定的预测价值。 Objective To dynamically monitor the levels of procalcitoninogen(PCT)and interleukin-6(IL-6)in oncology patients with concurrent infections in the intensive care unit(ICU),and to assess the predictive value of PCT and IL-6 on the prognosis of patients with oncology concurrent infections in the ICU.Methods 208 patients with tumourcomplicated infections admitted to the ICU from July 2022 to December 2023 were selected as the study subjects,and were divided into 163 cases in the survival group and 45 cases in the death group according to the survival prognosis during the stay in the ICU.Blood specimens were collected from all patients on the lst,3rd,5th and 7th days of ICU admission to test serum PCT and plasma IL-6,to compare the differences in the levels of the two indexes between the two groups,and ROC curves were plotted to assess the predictive value of single and combined testing of the two indexes on the prognosis of patients with tumour-complicated infections.Results The percentage of patients who were mechanically ventilated and developed shock in the death group was significantly higher than that in the survival group,and the differences were all statistically significant(P<0.05);the differences in the levels of PCT and IL-6 on day 7 between the death group and the survival group were all statistically significant(P<O.05);by ROC curve analysis,PCT,IL-6 and the combination of PCT,IL-6 and the combination of PCT,IL-6 and the combination of PCT and IL-6 on day 7 had a better predictive efficacy for survival and death in patients with tumour complications and infections,and the differences were all statistically significant(P<0.05).better predictive efficacy,and the differences were all statistically significant(P<0.05);the AUC areas for predicting death were 0.679,0.664 and 0.699,with sensitivities of 75.6%,60%and 77.8%,and specificities of 61.3%,63%and 60.7%,respectively.Conclusion Dynamic monitoring of PCT and IL-6 levels has a predictive value for survival prognosis of patients with tumour-complicated infections in the ICU.
