摘要
目的研究和总结国医大师及全国名中医对高血压的证治用药规律。方法收集国医大师和全国名中医治疗高血压的医案,建立相应的经验文献数据库,运用Excel进行频数统计、SPSS聚类分析和R Studio关联规则分析等数据挖掘方法分析中药处方用药规律,使用Origin进行绘图。结果筛选出的166首处方中,共涉及中药239味,用药频数2166次,其中使用频数≥30的中药有24味,天麻、牛膝、钩藤、茯苓、丹参、甘草、半夏、白芍、川芎和石决明居前10位。以活血化瘀药、平肝息风药、清热药、补气药、解表药出现频数最高,累计频率为53.37%。药性以寒、温为主,药味以甘、苦为主。同时挖掘出5类聚类组合核心方剂、4组常用药对及7种三味药物有效关联规则。结论国医大师及全国名中医治疗高血压以平肝潜阳为核心,运用经典平肝息风方剂的同时注重从气血论治,用药灵活、整体兼顾、标本兼治,并积极处理瘀血、水饮等兼证。
Objectivee To study and summarize the treatment principles and herbal prescription patterns of Traditional Chinese Medicine(TCM)Master and national famous TCM practitioner for hypertension.Methods Case records of hypertension treatment by TCM Masters and National famous TCM practitioners were collected to establish an experience-based literature database.Data mining methods,including frequency statistics using Excel,cluster analysis using SPSS,and association rule analysis using R Studio,were applied to analyze the herbal prescription patterns.Visualization was conducted using Origin.Results A total of 166 prescriptions involving 239 herbs,with a cumulative frequency of 2166 uses were analyzed.Among these,24 herbs had a usage frequency≥30,with the top 10 being Gastrodiae Rhizoma,Achyranthis Bidentatae Radix,Uncariae Ramulus cum Uncis,Poria,Salviae Miltiorrhizae Radix et Rhizoma,Glycyrrhizae Radix et Rhizoma,Pinelliae Rhizoma,Paeoniae Radix Alba,Chuanxiong Rhizoma and Haliotidis Concha.Herbs that promoted blood circulation and remove blood stasis,calmed the liver and extinguish wind,clear heat,tonify qi,and relieved the exterior appeared most frequently,accounting for a cumulative frequency of 53.37%.The properties of the herbs were primarily cold and warm,while the flavors were predominantly sweet and bitter.Additionally,5 core formula clusters,4 commonly used herbal pairs,and 7 effective association rules involving 3 herbs were identified.Conclusion The treatment of hypertension by TCM Masters and national famous TCM practitioners focused on calming the liver and subduing yang as the core principle.While employing classical formulas for calming the liver and extinguishing wind,they emphasize treating based on qi and blood theory.Their herbal prescriptions are flexible,holistic and address both root causes and symptoms,actively managing concurrent conditions such as blood stasis andretained fluids.
