摘要
目的探究黑色素瘤相关抗原D2(MAGED2)、上皮细胞膜蛋白2(EMP2)表达与H3K27M突变型弥漫性中线胶质瘤(DMG)预后的关系。方法回顾性分析武汉市红十字会医院2021年12月至2023年12月收治的117例H3K27M突变型DMG术后患者的临床资料,比较肿瘤组织及其癌旁正常组织MAGED2、EMP2的表达,多因素分析采用Cox比例风险回归模型,采用KaplanMeier法绘制生存曲线,并进行Log-rank检验分析MAGED2、EMP2表达与患者预后的关系。结果肿瘤组织中MAGED2、EMP2的阳性率均高于癌旁正常组织(P<0.05);MAGED2阳性率、EMP2阳性率在肿瘤部位、WHO分级、有无转移方面差异均有统计学的意义(P<0.05);本研究中117例H3K27M突变型DMG患者共进行了3~36个月的随访,Kaplan-Meier生存分析显示,MAGED2阳性患者的生存时间低于阴性患者(χ^(2)=65.356,P=0.000),EMP2阳性患者的生存时间低于阴性患者(χ^(2)=54.578,P=0.000);单因素Cox回归模型分析显示,WHO分级、有无转移、MAGED2阳性、EMP2阳性是H3K27M突变型DMG患者无进展生存的影响因素(P<0.05);多因素Cox回归模型分析显示,WHO分级、有无转移、MAGED2阳性、EMP2阳性均是3K27M突变型DMG患者预后的独立危险因素。结论MAGED2、EMP2在H3K27M突变型DMG中的阳性表达与患者临床病理特征相关,并提示患者生存时间较短,可作为评估H3K27M突变型DMG患者预后的潜在生物学标志物。
Objective To investigate the relationship between the expression of melanomaassociated antigen D2(MAGED2)and epithelial membrane protein 2(EMP2)and the prognosis of H3K27M-mutant diffuse midline glioma(DMG).Methods A retrospective analysis was conducted on the clinical data of 117 patients with H3K27M-mutant DMG who were admitted to the hospital from December 2021 to December 2023.The expressions of MAGED2 and EMP2 in tumor tissues and adjacent normal tissues were compared.Cox proportional hazards regression model was used for multivariate analysis and Kaplan-Meier survival curves were plotted.Logrank test was used to analyze the relationship between the expressions of MAGED2 and EMP2 and the prognosis.Results The positive rates of MAGED2 and EMP2 in tumor tissues were higher than those in adjacent normal tissues(P<0.05).There were statistically significant differences in the positive rates of MAGED2 and EMP2 in terms of tumor location,WHO grading,and metastasis(P<0.05).All patients enrolled in the study were followed up for 3-36 months.Kaplan-Meier survival analysis showed that survival time of MAGED2 positive patients was shorter than that of negative ones(χ^(2)=65.356,P=0.000),and survival time of EMP2 positive patients was shorter than that of negative ones(χ^(2)=54.578,P=0.000).Univariate Cox regression analysis showed that WHO grading,metastasis,MAGED2 positivity,and EMP2 positivity were factors influencing progression-free survival of patients with H3K27M-mutant DMG(P<0.05).Multivariate Cox regression analysis showed that WHO grading,metastasis,MAGED2 positivity,and EMP2 positivity were independent risk factors for the prognosis of patients with 3K27Mmutant DMG.Conclusions MAGED2 and EMP2 are positive in H3K27M-mutant DMG and related to the patient’s clinicopathological features.Moreover,MAGED2 positivity and EMP2 positivity indicate shorter survival time,and they can serve as potential biological markers for evaluating the prognosis of patients with H3K27M-mutant DMG.
