摘要
四逆散作为常用经方,组方精炼,配伍恰当,具有疏理气机、透达郁阳之效,应用广泛。检索整理近20年文献,从方证探幽、临床运用、机制研究总结四逆散在消化系统肿瘤中的应用,发现本方基础方证具有调和中土、疏肝养木、调理气机等特点;在临床运用方面,1)可用于消化系统癌前病变,逆转恶性肿瘤演变进程;2)可直接治疗消化系统肿瘤;3)可缓解癌性疼痛、肿瘤相关性抑郁与失眠等肿瘤常见并发症;4)可减轻消化系统肿瘤因西医治疗产生的不良反应,包括手术不良反应和化疗、靶向的不良反应等,提高生命质量。在作用机制研究方面,四逆散具有改善细胞损伤,减轻炎症状态;抑制肿瘤细胞增殖,促进肿瘤细胞凋亡;抗血管生成,抑制肿瘤转移等作用。无论是临床研究还是作用机制研究,四逆散治疗消化系统肿瘤均符合中医“治未病”理论。现通过文献检索和归纳总结,系统综述四逆散在消化系统肿瘤的研究现状,并总结了目前临床研究与实验机制研究的不足之处,以期为后续深入研究经方防治肿瘤疾病提供理论支持和新思路。
With a refined formula and appropriate combination,Sini San,a commonly used classical formula,has the effects of regulating qi movement and resolving stagnated yang.In this paper,the literature of nearly 20 years was collated to summarize the application of Sini San in digestive system tumors from the perspectives of evidence exploration,clinical application,and mechanism research.It is found that this formula has the characteristics of regulating and harmonizing the Middle Earth,dredging the liver and nourishing the wood,as well as regulating the qi movement.In terms of clinical application,Sini San has the following functions.1)Treating pre-cancerous lesions in the digestive system,reversing the malignant tumor evolution.2)Directly treating digestive system tumors.3)Alleviating cancer pain,depression,insomnia,and other common complications of tumors.4)Alleviating the adverse reactions of digestive system tumors due to Western medical treatments,including the surgery side effects,and adverse reactions of chemotherapies and targeted therapies,to improve life quality.In mechanism research,Sini San has the effect of improving cell damage,reducing inflammation,inhibiting tumor cell proliferation,promoting tumor cell apoptosis,anti-angiogenesis,and inhibiting tumor metastasis.In both clinical and mechanistic research,the treatment of digestive system tumors with Sini San is in line with the traditional Chinese medicine(TCM) theory of “preventive treatment of disease”.Through literature search and summarization,this paper systematically reviews the current research status of Sini San in digestive system tumors,and summarizes the shortcomings of the current clinical research and experimental mechanism research,with a view to providing theoretical support and new ideas for the subsequent in-depth research on the prevention and treatment of tumor by classical prescription.
