摘要
目的:探讨子宫内膜息肉患者行宫腔镜切除术后放置曼月乐环在预防中远期息肉复发的效果。方法:选取2022年4月—2023年5月在南平市妇幼保健院接受宫腔镜下子宫内膜息肉电切手术患者104例,根据术后治疗方案分为对照组(n=49例)和观察组(n=55例)。对照组术后口服屈螺酮炔雌醇片,观察组术后放置曼月乐环。随访12个月,比较两组患者子宫内膜厚度、出血情况、性激素水平、不良反应及复发率。结果:两组患者术后1个月和术后6个月子宫内膜厚度及失血量评估图(PBAC)评分低于术前,且术后6个月低于术后1个月,差异有统计学意义(P<0.05)。观察组患者术后1个月和6个月子宫内膜厚度及PBAC评分低于对照组,差异有统计学意义(P<0.05)。两组术后6个月促卵泡激素(FSH)、雌二醇(E2)、孕酮(P)均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组不良反应率和复发率低于对照组,差异有统计学意义(P<0.05)。结论:在子宫内膜息肉经宫腔镜切除术治疗后放置曼月乐环可有效改善患者的性激素水平和子宫内膜厚度,并降低中远期的复发风险。
Objective:To explore the effect of placing manyuele in preventing the expected recurrence of endometrial polyps after hysteroscopic resection.Methods:From April 2022 to May 2023,104 patients who underwent hysteroscopic endometrial polypectomy in nanping maternal and child health hospital were divided into observation group(55 cases)and control group(49 cases)according to the different postoperative treatment schemes of the two groups.The control group took drospirenone ethinylestradiol tablets orally after operation,and the observation group placed Manyuele ring after operation.The prognosis of the two groups was compared after 12 months of follow-up.Results:The endometrial thickness and blood loss assessment chart(PBAC)scores in the observation group were lower than those in the control group at 1 month and 6 months after operation,with statistical significance(P<0.05).Follicle stimulating hormone,estradiol and progesterone in the observation group were lower than those in the control group 6 months after operation,and the difference was statistically significant(P<0.05).The adverse reaction rate and recurrence rate in the observation group were lower than those in the control group,with statistical significance(P<0.05).Conclusion:Placement of manyuele after hysteroscopic resection of endometrial polyps can effectively improve the sex hormone level and endometrial thickness of patients and reduce the risk of recurrence in the medium and long term.
