摘要
目的观察水陆二仙丹合桃核承气汤加减方治疗糖尿病肾病的临床效果。方法选取2023年1月—2024年1月成都中医药大学附属绵阳医院收治的糖尿病肾病患者80例,依据随机数字表法分为观察组(n=40)和对照组(n=40),对照组给予厄贝沙坦片治疗,观察组在对照组基础上给予水陆二仙丹合桃核承气汤加减方治疗,2组均治疗8周。比较2组临床疗效、治疗前后血糖指标(空腹血糖、糖化血红蛋白)、血脂指标[总胆固醇(TC)、三酰甘油(TG)]、肾功能指标[血尿素氮(BUN)、血肌酐(SCr)、估算肾小球滤过率(eGFR)、尿白蛋白—肌酐比值]、中医证候积分,不良反应。结果观察组总有效率为97.50%,高于对照组的85.00%(χ^(2)=3.914,P=0.048)。治疗8周后,2组空腹血糖、糖化血红蛋白、TC及TG水平均较治疗前降低,且观察组降低幅度大于对照组(P<0.01);2组BUN、SCr、尿白蛋白—肌酐比值均较治疗前降低,eGFR均较治疗前升高,且观察组降低/升高幅度大于对照组(P<0.01);2组中医证候积分均较治疗前降低,且观察组降低幅度大于对照组(P<0.01)。观察组与对照组不良反应总发生率比较差异无统计学意义(5.00%vs.7.50%,χ^(2)=0.213,P=0.644)。结论水陆二仙丹合桃核承气汤加减方治疗糖尿病肾病的效果较好,可改善患者血糖、血脂、肾功能指标,降低中医证候积分,安全性较高。
Objective To observe the clinical effect of Shuilu Erxian dan combined with Taohe Chengqi decoction modified in the treatment of diabetic nephropathy.Methods A total of eighty patients with diabetic nephropathy admitted to Mianyang Hospital Affiliated to Chengdu University of Traditional Chinese Medicine from January 2023 to January 2024 were selected and divided into observation group(n=40)and control group(n=40)according to the random number table method.The control group was treated with irbesartan tablets,and the observation group was treated with Shuilu Erxian dan combined with Taohe Chengqi decoction modified on the basis of the control group.Both groups were treated for 8 weeks.The clinical efficacy,blood glucose indicators(fasting blood glucose,glycated hemoglobin),blood lipid indicators(TC,TG),renal function indicators(BUN,SCr,eGFR,urine albumin-creatinine ratio),TCM syndrome score before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 97.50%,which was higher than 85.00%of the control group(χ^(2)=3.914,P=0.048).After 8 weeks of treatment,the levels of fasting blood glucose,glycated hemoglobin,TC and TG in both groups were lower than those before treatment,and the reduction amplitude in the observation group was greater than that in the control group(P<0.01).In both groups,BUN,SCr and urine albumin-creatinine ratio were decreased compared with those before treatment,eGFR were increased compared with that before treatment,the decrease/increase amplitude in the observation group was greater than that in the control group(P<0.01).The TCM syndrome scores of both groups were lower than those before treatment,and the reduction amplitude of the observation group was greater than that of the control group(P<0.01).There was no statistically significant difference in the total incidence of adverse reactions between the observation group and the control group(5.00%vs.7.50%,χ^(2)=0.213,P=0.644).Conclusion Shuilu Erxian dan combined with Taohe Chengqi decoction modified has a relatively good therapeutic effect on diabetic nephropathy.It can improve the patient′s blood sugar,blood lipid and renal function indicators,lower the TCM syndrome score,and has a relatively high safety.
