Leptin、CXCL4和NK细胞在冠心病中的相关性分析

摘要 目的:探讨瘦素(Leptin)、CXC趋化因子配体4(CXC chemokine ligand 4,CXCL4)以及自然杀伤(natural killer,NK)细胞在冠状动脉粥样硬化性心脏病(coronary atherosclerotic heart disease,CHD)发展进程中的相关性。方法:选取2023年1—5月在南京医科大学附属江宁医院住院的CHD患者70例,分为急性冠脉组和慢性冠心病组;另选择同期健康体检者30例为健康对照组。采用流式细胞仪检测急性冠脉组、慢性冠心病组以及健康对照组外周血中NK(CD45+CD3-CD19-CD56+)细胞比例与人Th1/Th2/Th17亚群(IL-6、IFN-γ)的表达;采用ELISA检测急性冠脉组、慢性冠心病组与健康对照组血清中Leptin和CXCL4的表达水平;SPSS 19.0分析血清Leptin、CXCL4表达水平与NK细胞在CHD患者疾病发展进程中的相关性。结果:急性冠脉组、慢性冠心病组患者外周血NK细胞的占比及炎症因子IL-6、IFN-γ的表达显著高于健康对照组(P<0.05);急性冠脉组、慢性冠心病组患者血清中CXCL4和Leptin表达高于健康对照组(P<0.05);Pearson分析提示CHD患者血清Leptin、CXCL4的表达与NK细胞比例呈正相关(r=0.45、0.55,均P<0.001)。结论:Leptin和CXCL4可能存在蛋白互作,促进NK细胞活化,参与CHD疾病的进展。 Objective:To investigate the correlation between Leptin,CXC chemokine ligand 4(CXCL4),and natural killer(NK)cells in the progression of coronary atherosclerotic heart disease(CHD).Methods:A total of 70 patients with CHD admitted to the Jiangning Hospital Affiliated to Nanjing Medical University from January to May 2023 were selected and divided into an acute coronary group and a chronic coronary heart disease group.Additionally,30 healthy individuals who underwent physical examinations during the same period were chosen as the healthy control group.The proportion of NK(CD45+CD3-CD19-CD56+)cells and the expression of human Th1/Th2/Th17 subsets(IL-6,IFN-γ)in peripheral blood were detected using a flow cytometer in the acute coronary group,chronic coronary heart disease group,and healthy control group.The expression levels of Leptin and CXCL4 in serum were measured by ELISA in the acute coronary group,chronic coronary heart disease group,and healthy control group.The correlation between the expression levels of serum Leptin and CXCL4 and the proportion of NK cells in the disease progression of CHD patients was analyzed using SPSS 19.0.Results:The proportion of NK cells in peripheral blood and the expression of inflammatory factors IL-6 and IFN-γwere significantly higher in the acute coronary group and chronic coronary heart disease group compared with the healthy control group(P<0.05).The expression levels of CXCL4 and Leptin in serum were higher in the acute coronary group and chronic coronary heart disease group than those in the healthy control group(P<0.05).Pearson analysis indicated that the expression of serum Leptin and CXCL4 was positively correlated with the proportion of NK cells in CHD patients(r=0.45,0.55,both P<0.001).Conclusion:Leptin and CXCL4 may interact with each other,promoting the activation of NK cells and contributing to the progression of CHD.
