摘要
目的:探究心脏超声造影联合血清肌钙蛋白T(cTnT)、可溶性生长刺激表达基因2蛋白(sST2)评估冠心病(CHD)患者冠状动脉病变程度的临床价值。方法:选取2022年1月至2024年1月在新乡市第一人民医院就诊的CHD患者182例为观察组,另选取同期在新乡市第一人民医院体检的健康志愿者50例为对照组。两组均实施心脏超声造影检查,获取最大微泡数量(A)与造影剂填充速度(β)等参数,并进行血清cTnT、sST2检测。依据Gensini评分判定冠状动脉病变程度,比较不同冠状动脉病变程度患者A值、β值、cTnT及sST2水平,绘制受试者工作特征(ROC)曲线,评估上述指标对重度冠状动脉病变的诊断价值。结果:观察组A值、β值均低于对照组(P <0.05),其cTnT、sST2水平均高于对照组(P <0.05)。轻度冠状动脉病变患者A值、β值高于中度冠状动脉病变、重度冠状动脉病变患者(P <0.05),中度冠状动脉病变患者A值、β值高于重度冠状动脉病变患者(P <0.05);轻度冠状动脉病变患者cTnT、sST2水平低于中度冠状动脉病变、重度冠状动脉病变患者(P <0.05),中度冠状动脉病变患者cTnT、sST2水平低于重度冠状动脉病变患者(P <0.05)。A值、β值、cTnT、sST2水平单独及联合检测诊断重度冠状动脉病变的曲线下面积(AUC)分别为0.819、0.832、0.755、0.853和0.981(P <0.05)。结论:心脏超声造影参数A值、β值及血清cTnT、sST2水平均与CHD患者冠状动脉病变的程度存在关联,上述指标联合检测在重度冠状动脉病变的评估中具有良好的应用价值。
Objective:To explore the clinical value of contrast-enhanced echocardiography combined with serum cardiac troponin T(cTnT)and soluble suppression of tumorigenicity 2 protein(sST2)in assessing coronary artery lesion severity in patients with coronary heart disease(CHD).Methods:A total of 182 CHD patients admitted to Xinxiang First People’s Hospital from January 2022 to January 2024 were enrolled as the observation group,and 50 healthy volunteers undergoing physical examinations during the same period were selected as the control group.Both groups underwent contrast-enhanced echocardiography to obtain parameters such as maximum microbubble count(A)and contrast agent filling rate(β),along with serum cTnT and sST2 detection.Coronary artery lesion severity was determined using the Gensini score.Parameters(A,β)and serum markers(cTnT,sST2)were compared among patients with mild,moderate,and severe lesions.Receiver operating characteristic(ROC)curves were plotted to evaluate the diagnostic efficacy of these indicators for severe coronary artery lesions.Results:The observation group exhibited lower A andβvalues but higher cTnT and sST2 levels compared to the control group(P<0.05).Patients with mild lesions had higher A andβvalues than those with moderate or severe lesions,while moderate lesion patients showed higher A andβvalues than severe lesion patients(P<0.05).Conversely,cTnT and sST2 levels were lower in mild lesion patients than in moderate or severe lesion patients,and lower in moderate lesion patients than in severe lesion patients(P<0.05).The areas under the ROC curve(AUCs)for A,β,cTnT,sST2 alone,and their combination in diagnosing severe coronary lesions were 0.819,0.832,0.755,0.853,and 0.981,respectively(P<0.05).Conclusion:Contrast-enhanced echocardiography parameters(A,β)and serum cTnT and sST2 levels are correlated with the severity of coronary artery lesions in CHD patients.Combined detection of these markers demonstrates excellent clinical value in evaluating severe coronary artery lesions.
