摘要
目的:研究急性尿潴留(AUR)大鼠尿动力学及膀胱组织形态学特征。方法:12周龄雌性SD大鼠32只随机分为4组,每组8只。其中3组采用过度充盈膀胱1 h的方法建立AUR模型,分别于造模后2 h、1 d、7 d进行尿动力学检测,取膀胱组织行HE、Masson、免疫组化染色观察;余1组为正常对照。结果:与正常组比较,AUR各组膀胱功能降低(P<0.05),黏膜下层血管数量、血管直径均减小(P<0.05);AUR 1 d、AUR 7 d组逼尿肌层血管数量和直径减小,神经纤维直径减小(P<0.05);4组神经纤维数量差异无统计学意义(P>0.05)。与AUR 1 d和7 d组比较,AUR 2 h组逼尿肌不稳定收缩次数增多,膀胱容量减低,膀胱顺应性减小,排尿后压力升高(P<0.05)。与正常组和AUR 2 h组比较,AUR 7 d组膀胱黏膜上皮层厚度增大,逼尿肌层厚度减小,逼尿肌层胶原纤维比例增大(P<0.05)。结论:AUR 2 h大鼠膀胱功能最差,膀胱黏膜层及黏膜下层的损伤早于逼尿肌层,AUR 7 d膀胱功能及组织损伤未恢复正常。
Aim:To study the urodynamics and bladder histomorphological characteristics of acute urinary retention(AUR)model rats.Methods:A total of 3212-week-old female SD rats were randomly allocated into 4 groups and 8 in each group.Three groups were used to establish AUR model by overfilling the bladder with normal saline for 1 hour,and then received urodynamic tests,HE,Masson,immunohistochemical staining observation of bladder at 2 hours,1 day,and 7 days after modeling,respectively,and named as AUR 1 h group,AUR 2 h group,and AUR 7 d group.The remaining group was the normal control.Methods:Compared with the normal control group,the bladder function of each AUR group decreased(P<0.05),the number and diameter of submucosal blood vessels decreased(P<0.05);the number and diameter of blood vessels in the detrusor muscle layer decreased in the AUR 1 d group and AUR 7 d group,and the diameter of nerve fibers decreased(P<0.05);there was no significant difference in the number of nerve fibers among the 4 groups(P>0.05).Compared with the AUR 1 d and 7 d groups,the AUR 2 h group showed an increase in the frequency of detrusor instable contraction,a decrease in bladder volume capacity,a decrease in bladder compliance,and an increase in post-void pressure(P<0.05).Compared with the normal group and AUR 2 h group,the AUR 7 d group showed an increase in epithelial layer thickness,a decrease in detrusor muscle layer thickness,and an increase in the proportion of collagen fibers in the detrusor muscle layer(P<0.05).Conclusion:The bladder function in AUR 2 h rats is the worst.The injury of the mucosa and submucosa in the bladder is earlier than the muscle layer.The bladder function and structures do not recover 7 days after modeling.
