胸部增强CT与3D导航软件DeepInsight运行分析及临床应用

摘要 目的探讨胸部增强CT技术操作与3D导航软件DeepInsight运行关系。方法收集2018年10月至2023年1月南通大学附属医院肺结节手术患者108例胸部增强CT DICOM 0.625 mm重建薄层图像数据,应用医学影像存档与通讯系统(PACS)测量记录主动脉CT值、肺动脉CT值及主动脉-肺动脉CT差值。依据DeepInsight软件运行结果,对测定纪录的主动脉CT值、肺动脉CT值、主动脉-肺动脉CT差值分为满意与不满意两组,并进行统计学分析。胸外科手术中应用运行满意DeepInsight 3D图像进行手术导航并观察与手术解剖是否一致。结果主动脉CT值、肺动脉CT值及主动脉-肺动脉CT差值满意组87例(80.6%),不满意组21例(19.4%)。满意组与不满意组对比:主动脉CT值分别为(442.11±74.05)HU、(357.48±58.29)HU(P=0.09),肺动脉CT值分别为(163.48±36.86)HU、(226.71±125.02)HU(P<0.01),主动脉-肺动脉CT差值分别为(278.59±85.62)HU、(130.76±149.12)HU(P=0.03)。统计分析表明,肺动脉CT值偏高以及主动脉-肺动脉CT差值偏低与DeepInsight软件运行不满意有关。DeepInsight软件运行满意组3D导航图像清晰,与手术实际解剖对照一致。结论胸部增强CT正确掌握主动脉CT值、肺动脉CT值及主动脉-肺动脉CT差值,减少增强CT技术操作失误是3D导航软件DeepInsight成功运行的关键。 【Objective】To explore the relationship between chest enhanced CT scan and 3D navigation software DeepInsight operation.【Methods】From October 2018 to January 2023,a total of 108 of pulmonary nodule surgery cases treated in Affiliated Hospital of Nantong University were involved in this study.The DICOM 0.625 mm thin-layer reconstruction image data from enhanced chest CT scan were collected and the CT values of aorta,pulmonary artery and the difference values between aorta and pulmonary artery were measured and recorded by the medical picture archiving and communication system(PACS).According to the results of DeepInsight software operation,the aorta CT values,pulmonary artery CT values and the difference in CT values between aorta and pulmonary artery recorded were divided into two groups:satisfied and dissatisfied,and statistical analysis was carried out.The satisfactory DeepInsight 3D images were applied in thoracic surgery and compared with surgical anatomical consistency.【Results】There were 87 cases in satisfied group(80.6%),and 21 cases in dissatisfied group(19.4%).Comparison between these two groups,the aorta CT values were 442.11±74.05 HU and 357.48±58.29 HU(P=0.09),the pulmonary artery CT values were 163.48±36.86 HU and 226.71±125.02 HU(P<0.01),and the difference in CT values between aorta and pulmonary artery were 278.59±85.62 HU and 130.76±149.12 HU(P=0.03)respectively.Statistical analysis showed that high pulmonary artery CT value and low difference in CT values between aorta and pulmonary artery were related to the unsatisfactory operation of DeepInsight software.The 3D navigation images of the satisfied group were clear and consistent with the actual anatomical controls of the surgery.【Conclusion】The key to successfully running 3D navigation software DeepInsight is to correctly grasp the aorta CT value,pulmonary artery CT value and the difference in CT values between aorta and pulmonary artery,and reduce the operation errors of enhanced CT scan.
