摘要
目的分析血清晚期糖基化终末产物(AGE)、胰高血糖素样肽-1(GLP-1)、长链非编码RNA转移相关肺腺癌转录本1(lncRNA MALAT1)对妊娠糖尿病(GDM)孕妇分娩低血糖新生儿的预测价值。方法将本院100例GDM孕妇分娩出的新生儿分为新生儿低血糖组60例和对照组(新生儿血糖正常)40例。比较两组及其母体的临床资料、血清AGE、GLP-1、l ncRNA MALAT1表达水平。分析GDM孕妇分娩低血糖新生儿的影响因素及血清AGE、GLP-1、lncRNA MALAT1表达水平与血糖指标[空腹血糖(FBG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbAlc)]的相关性。采用ROC曲线分析血清AGE、GLP-1、lncRNA MALAT1对GDM孕妇分娩低血糖新生儿的预测价值。结果两组孕妇孕前超重或肥胖、血糖控制良好及新生儿早产、低体质量、低体温、规范喂养比较,差异均有显著性(P<0.05)。新生儿低血糖组FBG、2hPG、HbAlc、AGE水平高于对照组,GLP-1、lncRNA MALAT1表达水平低于对照组(P<0.05)。孕前超重或肥胖、血糖控制良好及新生儿早产、低体质量、低体温、规范喂养、血糖指标、血清AGE、GLP-1、lncRNA MALAT1均为GDM孕妇分娩低血糖新生儿的影响因素(P<0.05)。GDM孕妇血清AGE与FBG、2hPG、HbAlc呈正相关(P<0.001),GLP-1、lncRNA MALAT1与FBG、2hPG、HbAlc呈负相关(P<0.001)。血清AGE、GLP-1、lncRNA MALAT1联合预测GDM孕妇分娩低血糖新生儿的AUC值高于其单独指标预测的AUC值,具有良好预测效能。结论血清AGE、GLP-1、lncRNA MALAT1对GDM孕妇分娩低血糖新生儿具有良好预测效能。
Aim To analyze the predictive value of serum advanced glycation end product(AGE),glucagon-like peptide-1(GLP-1),long non-coding RNA metastasis-associated lung adenocarcinoma transcript 1(lncRNA MALAT1)for gestational diabetes mellitus(GDM)pregnant women delivering hypoglycemic newborn.Methods The newborns of 100 GDM pregnant women in our hospital were divided into a neonatal hypoglycemia group of 60 cases and a control group of 40 cases(with normal neonatal blood sugar).The clinical data,serum AGE,GLP-1,and lncRNA MALAT1 expression levels of two groups of GDM pregnant women were compared.The influencing factors of delivering hypoglycemic newborns from GDM pregnant women and the correlation between serum AGE,GLP-1,lncRNA MALAT1 expression levels and blood glucose indicators[fasting blood glucose(FBG),2 h postprandial blood glucose(2hPG),glycated hemoglobin(HbAlc)]were analyzed.ROC curve analysis was used to evaluate the predictive value of serum AGE,GLP-1,and lncRNA MALAT1 for delivering hypoglycemic newborns from GDM pregnant women.Results There were significant differences(P<0.05)in the pre-pregnancy overweight or obesity,blood glucose control effect,premature birth,low birth weight,low body temperature,and feeding norms between the two groups of pregnant women.The levels of FBG,2hPG,HbAlc,and AGE in the neonatal hypoglycemia group were higher than those in the control group,while the expression levels of GLP-1 and l ncRNA MALAT1 were lower than those in the control group(P<0.05).Pre-pregnancy overweight or obesity,effective blood glucose control,premature birth,low body weight,hypothermia,standardized feeding,blood glucose indicators,serum AGE,GLP-1,and lncRNA MALAT1 were all influencing factors for GDM pregnant women to delivery hypoglycemic newborns(P<0.05).Serum AGE in GDM pregnant women were positively correlated with FBG,2hPG,and HbAlc(P<0.001),while GLP-1 and lncRNA M ALAT1 were negatively correlated with FBG,2hPG,and HbAlc(P<0.001).The AUC of predicting GDM pregnant women to delivery hypoglycemic newborns by combining serum AGE,GLP-1,and lncRNA MALAT1 is higher than the individual prediction value,indicating good predictive efficacy.Conclusion Serum AGE,GLP-1,and lncRNA MALAT1 have good predictive efficacy for delivering hypoglycemic newborns from GDM pregnant women.
