摘要
目的观察人免疫球蛋白与干扰素联用方案对肠道病毒71型(EV71)感染致重症手足口病患儿免疫功能及外周血炎症因子水平的影响。方法选取本院120例EV71型感染致重症手足口病患儿为研究人群,采用抽签法均分为干扰素组和联合组。两组均采用抗感染、退热、调节水电解质失衡及补液等常规治疗,干扰素组在常规治疗的基础上雾化吸入干扰素,联合组在干扰素组基础上联用人免疫球蛋白治疗。比较两组治疗前后外周血炎症因子白细胞介素(IL)-6、IL-8、内皮素(ET)-1、γ干扰素诱导蛋白(IP)-10、免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平。比较两组临床疗效、症状消失时间、住院时间、不良反应发生率。结果治疗后,两组IL-6、IL-8、ET-1、IP-10水平及CD8^(+)值均降低,且联合组低于干扰素组(P<0.05);两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均升高,且联合组高于干扰素组(P<0.05)。联合组总有效率高于干扰素组(P<0.05),症状消失时间和住院时间均短于干扰素组(P<0.05)。两组不良反应比较,差异无显著性(P>0.05)。结论人免疫球蛋白联用干扰素可改善EV71型感染致重症手足口病患儿免疫功能,降低炎症因子水平,提高临床疗效。
Aim To investigate the effects of human immunoglobulin combined with interferon on immune function and inflammatory factors in children with severe hand-foot-mouth disease caused by enterovirus 71(EV71)infection.Methods 120 children with severe EV71-induced hand-foot-mouth disease in our hospital were selected as the study population,and were divided into interferon group and combined group by alottery method.Both groups received conventional treatments such as anti-infection,fever reduction,regulation of water and electrolyte imbalance,and fluid replacement.The interferon group received nebulized interferon inhalation on the basis of conventional treatment,while the combined group received combined treatment with human immunoglobulin on the basis of interferon group.The levels of peripheral inflammatory cytokines interleukin-6(IL-6),IL-8,endothelin-1(ET-1),interferonγ-inducible protein-10(IP-10),and immune function indicators(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))before and after treatment between the two groups were compared.The clinical efficacy,symptom disappearance time,hospitalization time,and incidence of adverse reactions between the two groups were compared.Results After treatment,the levels of IL-6,IL-8,ET-1,IP-10,and CD8^(+)values in both groups were decreased,and the combined group was lower than the interferon group(P<0.05);Both groups showed an increase in CD3^(+),CD4^(+),and CD4^(+)/CD8^(+),with the combined group being higher than the interferon group(P<0.05).The total effective rate of the combined group was higher than that of the interferon group(P<0.05),and the time for symptom disappearance and hospitalization were shorter than those of the interferon group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Human immunoglobulin combined with interferon can improve immune function,reduce inflammatory factors and enhance clinical efficacy in children with severe hand-foot-mouth disease caused by EV71 infection.
