摘要
目的探究血尿酸及富含半胱氨酸蛋白61(CYR61)水平与老年糖尿病患者骨密度、骨代谢指标的相关性。方法选取老年糖尿病患者232例,根据血尿酸水平分为高血尿酸组(n=109)和血尿酸正常组(n=123)。检测两组血尿酸、CYR61、椎体L1~L4骨密度、股骨颈骨密度、全髋骨密度及骨代谢指标Ⅰ型前胶原氨基端延长肽(PINP)、β-I型胶原羧基端交联肽(β-CTX)、25-羟基维生素D 3[25-(OH)D 3]、骨钙素(OCN),并进行相关性分析。结果高血尿酸组血尿酸水平高于血尿酸正常组,CYR61水平低于血尿酸正常组(P<0.05)。高血尿酸组L1~L4骨密度及全髋骨密度均低于血尿酸正常组(P<0.05)。高血尿酸组PINP、β-CTX、25-(OH)D 3、OCN均低于血尿酸正常组(P<0.05)。Pearson相关性分析显示,血尿酸与全髋骨密度、L1~L4骨密度、25-(OH)D 3、PINP、β-CTX、OCN呈负相关(P<0.05);CYR61与全髋骨密度、L1~L4骨密度、PINP、25-(OH)D 3、OCN呈正相关,与β-CTX呈负相关(P<0.05)。结论血尿酸水平和CYR61表达与老年糖尿病患者骨密度和骨代谢指标显著相关。
Aim To investigate the correlation between serum uric acid,cysteine-rich protein 61(CYR61)and bone mineral density,bone metabolism in elderly patients with diabetes.Methods A total of 232 elderly diabetic patients were divided into a hyperuricemia group(n=109)and a normal uric acid group(n=123)according to serum uric acid levels.Serum uric acid,CYR61,lumbar spine(L1-L4)bone mineral density,femoral neck bone mineral density,total hip bone mineral density and bone metabolism markers(procollagen type I N-terminal propeptide(PINP),β-C-terminal telopeptide of type I collagen(β-CTX),25-hydroxyvitamin D3(25-(OH)D3),osteocalcin(OCN))were measured and analyzed.Results The hyperuricemia group had higher serum uric acid but lower CYR61 levels compared to the normal group(P<0.05).L1-L4 and total hip bone mineral density were lower in hyperuricemia group(P<0.05).PINP,β-CTX,25-(OH)D3 and OCN were lower in hyperuricemia group(P<0.05).Pearson analysis showed serum uric acid was negatively correlated with total hip BMD,L1-L4 bone mineral density,25-(OH)D3,PINP,β-CTX and OCN(P<0.05).CYR61 was positively correlated with total hip BMD,L1-L4 bone mineral density,PINP,25-(OH)D3 and OCN,but negatively correlated withβ-CTX(P<0.05).Conclusion Serum uric acid and CYR61 are significantly correlated with bone mineral density and bone metabolism in elderly diabetic patients.
