摘要
勃起功能障碍合并阻塞性睡眠呼吸暂停综合征常以“阳痿”就诊,临证使用5型磷酸二酯酶抑制剂改善勃起功能,存在因阻塞性睡眠呼吸暂停综合征导致的氧饱和度恶化等风险。古今医家多从肝、肾、脾、心论治“阳痿”,亦有发挥从“肺”论治。本文结合勃起功能障碍合并阻塞性睡眠呼吸暂停综合征患者临床生理、病理表现,基于常见“气虚痰湿”证型,提出勃起功能障碍合并阻塞性睡眠呼吸暂停综合征以“气虚”为主;“痰湿”既为病理产物,又是致病因素;方用补中益气汤合二陈汤加减以补中益气、理气化痰,再佐使温补肾阳,治病求本,标本兼治,以期为临床辨治提供新思路。
Erectile dysfunction combined with obstructive sleep apnea hypopnea syndrome(OSAHS)often presented as"impotence"during medical consultations.The clinical use of type 5 phosphodiesterase inhibitors to improve erectile function has the risk of deterioration of oxygen saturation due to obstructive sleep apnea hypopnea syndrome.Most ancient and modern physicians treat"impotence"from the perspective of liver,kidney,spleen and heart,as well as from the perspective of"lung".On the premise of combining ED with OSAHS clinical physiology and pathology,based on the common"Qi deficiency and phlegm dampness"syndrome type,it is proposed that ED combined with OSAHS is mainly based on"Qi deficiency";"phlegm dampness"is not only a pathological product,but also a pathogenic factor.The prescription is modified by Buzhong Yiqi Decoction and Erchen Decoction which has the effects of tonifying middle Qi,regulating Qi,resolving phlegm and combined with warming kidney Yang.In order to provide new ideas for clinical differentiation and treatment,we should seek the root cause of the disease and treat both the symptoms and the root causes.
