摘要
目的探讨产时超声在分娩方式及分娩时限中的预测价值。方法选取2024年1月至2024年12月期间在我院分娩的孕妇200例作为研究对象,均实施阴道试产,并进行产时超声检查,对分娩过程中胎头位置、胎方位、宫颈扩张程度及胎头下降角度的超声评估,分析其与分娩结局的相关性。结果阴道分娩组与剖宫产组在胎头位置、胎方位及胎头下降角度上存在显著差异(P<0.05);Logistic回归分析显示,胎头下降角度(AoP)是预测阴道分娩的独立因素(OR=1.32,95%CI:1.12-1.56,P<0.01);ROC曲线分析显示,AoP预测阴道分娩的曲线下面积(AUC)为0.86,最佳截断值为120°;胎头位置与分娩时限呈显著负相关(r=-0.72,P<0.01),AoP与分娩时限呈显著负相关(r=-0.68,P<0.01);ROC曲线分析显示,AoP预测分娩时限的AUC为0.82,最佳截断值为110°。结论产时超声可有效预测分娩方式(阴道分娩或剖宫产)及分娩时限,为临床决策提供重要参考。
Objective To explore the predictive value of ultrasound during labor in the mode of delivery and time of delivery.Method 200 pregnant women who delivered in our hospital between January 2024 and December 2024 were selected as the study subjects,all of whom underwent vaginal trial of labor and underwent ultrasound examination during labor and delivery,and ultrasound evaluation of fetal head position,fetal orientation,degree of cervical dilatation,and the angle of descent of the fetal head during delivery were performed to analyze the correlation with the delivery outcome.Result There were significant differences between the vaginal delivery group and the cesarean delivery group in fetal head position,fetal orientation and angle of descent of the fetal head(P<0.05);logistic regression analysis showed that the angle of descent of the fetal head(AoP)was an independent predictor of vaginal delivery(OR=1.32,95%CI:1.12-1.56,P<0.01);and ROC curve analysis showed that the AoP predicted the vaginal delivery with an area under the curve(AUC)of 0.86 and an optimal cutoff value of 120°;head position showed a significant negative correlation with the time frame of delivery(r=-0.72,P<0.01),and AoP showed a significant negative correlation with the time frame of delivery(r=-0.68,P<0.01);the ROC curve analysis revealed that AoP predicted the time frame of delivery with an AUC of 0.82,and an optimal cutoff value of 110°.Conclusion Ultrasound during labor can effectively predict the mode of delivery(vaginal delivery or cesarean section)and the time limit of delivery,providing an important reference for clinical decision-making.
