摘要
目的:探讨中药顺气脐贴外治治疗妇科腹部术后腹胀的临床疗效。方法:运用随机数字表法,将90例符合诊断标准的妇科腹部术后腹胀患者分为3组,即对照A组、观察B组和观察C组,每组30例。A组患者术后接受新斯的明肌注,B组患者术后使用顺气脐贴进行神阙穴外敷治疗,C组患者术后同时接受新斯的明肌注与顺气脐贴外敷的联合疗法。观察3组患者在术后主要症状积分、肠鸣音恢复正常时间以及首次排气时间的差异。结果:干预后,与A组比较,B组和C组术后恶心、呕吐、腹痛、腹胀等症状减轻更明显,且差异具有统计学意义(P<0.05);C组腹部术后肠鸣音恢复时间和首次排气时间均表现出显著的优势,差异具有统计学意义(P<0.01)。此外,与B组比较,C组腹胀、腹痛减轻效果更佳,但恶心呕吐症状缓解B组优于C组。干预后术后肠鸣音恢复和首次排气时间,B组、C组优于A组,差异具有统计学意义(P<0.01),B组与C组比较,差异无统计学意义。结论:中药外治法顺气脐贴的应用,在妇科腹部术后腹胀的治疗中展现出了良好的疗效。合理运用中药外治法,能够有效促进胃肠平滑肌蠕动,从而减轻术后腹胀及其伴随的腹痛、恶心、呕吐等症状,同时避免西药对胃肠道和循环系统的不良反应,提高患者的生活质量。
Objective:To explore the clinical efficacy of external treatment with traditional Chinese medicine Shunqi navel patch for postoperative abdominal distension in gynecological abdominal surgery.Methods:This study used a random number table method to divide 90 gynecological patients with postoperative abdominal distension who met the criteria into three groups:control group A,observation group B,and observation group C with 30 patients in each group.Patients in group A received intramuscular injection of neostigmine after operation;patients in group B were treated with Shunqi naval patch on Shenque acupoint after operation;patients in the group C received a combination therapy of neostigmine intramuscular injection and external application of Shunqi navel patch after surgery.The main symptom scores,the recovery time of bowel sounds and the first exhaust time were compared among the three groups.Results:After intervention,compared with group A,the symptoms of nausea,vomiting,abdominal pain,abdominal distension and other symptoms in group B and group C were alleviated more obviously,and the difference was statistically significant(P<0.05).The recovery time of bowel sound and the first exhaust time in group C showed significant advantages after abdominal surgery,and the difference was statistically significant(P<0.01).In addition,Compared with group B,the abdominal distension and abdominal pain in group C were better,but the nausea and vomiting symptoms in group B were better than those in group C.After intervention,the recovery time of bowel sounds and the first exhaust time in group B and group C were better than those in group A,and the differences were statistically significant(P<0.01).There was no statistically significant difference between group B and group C.Conclusion:The external treatment with traditional Chinese medicine,especially the application of Shunqi navel patch,has shown good efficacy in the treatment of abdominal distension after gynecological abdominal surgery.Reasonable use of traditional Chinese medicine external treatment can effectively promote the peristalsis of gastrointestinal smooth muscles,thereby significantly reducing postoperative abdominal distension and accompanying symptoms such as abdominal pain,nausea,and vomiting,while avoiding the side effects of Western medicine on the gastrointestinal and circulatory systems,greatly improving the quality of life of patients.
