摘要
冠状动脉粥样硬化性心脏病不稳定型心绞痛是一种常见的心绞痛类型,主要表现为胸前区疼痛反复发作,并且在频率和程度上较以往加重。中医将其归属于“胸痹”“心痛”等范畴。文章基于“阳化气,阴成形”理论,认为形成胸痹心痛的根本起因在于“阳化气”的功能不足,此乃构成不稳定型心绞痛的发病之本;“阴成形”的功能太过所形成的水饮、瘀血、痰浊停滞于心,共同构成不稳定型心绞痛的发病之标,此之谓胸痹,是阳气虚损、阴邪内聚的本虚标实病证,在治疗上应当采用温阳通痹的基本方法,选方为枳实薤白桂枝汤加减,以调整机体之阴阳,并结合临床病例对具体用药进行介绍。
Coronary heart disease unstable angina is a common type of angina pectoris,mainly manifested as recurrent pain in the anterior chest area,and in the frequency and degree of aggravation than before.In traditional Chinese medicine,it is classified under the categories of“chest paralysis”and“heart pain”.Based on the theory of“yang transforms qi and yin shapes it”,this paper believes that the root cause of chest paralysis and heart pain lies in the insufficiency of the function of“yang transforms qi”,which constitutes the pathogenesis of unstable angina pectoris,and the excess of the function of“yin shapes it”,which constitutes the pathogenesis of unstable angina pectoris.The function of“yin forming”is too much to form the water-drink,blood stasis,phlegm and turbidity stagnation in the heart,which constitutes the symptom of unstable angina pectoris,and this is called chest paralysis,which is the symptom of the disease of the deficiency of yang qi and the accumulation of yin evils,so the basic treatment should be warming yang,eliminating phlegm and removing blood stasis in the heart,and the prescription is Zhishi Xiebai Guizhi Decoction to regulate yin and yang,and the combination of the clinical cases on the specific use of drugs is introduced.The specific medication will be introduced with clinical cases.
