重度子痫前期患者血清同型半胱氨酸与血管内皮生长因子水平对胎盘功能及胎儿生长受限的预测价值 被引量:1

摘要 目的:探究重度子痫前期(Preeclampsia,PE)患者血清同型半胱氨酸(Homocysteine,Hcy)与血管内皮生长因子(Vascular endothelial growth factor,VEGF)水平对胎盘功能及胎儿生长受限的预测价值。方法:选取2020年7月-2024年7月期间我院妇产科收治的重度PE患者105例为研究对象。统计重度PE患者孕晚期胎盘功能以及胎儿生长受限情况,并将患者分为胎盘功能正常组与胎盘功能异常组;胎儿生长受限组与胎儿生长正常组。分别检测对比胎盘功能正常组与胎盘功能异常组的血清Hcy、VEGF水平以及胎儿生长受限组与胎儿生长正常组的血清Hcy、VEGF水平。绘制ROC特性曲线,计算Hcy、VEGF单独及联合检测对胎盘功能及胎儿生长受限的预测价值。结果:经孕晚期检查,105例重度PE患者,有74例患者胎盘功能异常纳入胎盘功能异常组,有31例患者胎盘功能正常则纳入胎盘功能正常组。105例重度PE患者中有54例患者胎儿生长受限纳入胎儿生长受限组,另外51例患者胎儿生长正常则被纳入胎儿生长正常组。胎盘功能异常组的血清Hcy水平显著高于胎盘功能正常组,胎儿生长受限组的血清Hcy水平显著高于胎儿生长正常组(P<0.05)。胎盘功能异常组的血清VEGF水平显著低于胎盘功能正常组,胎儿生长受限组的血清VEGF水平显著低于胎儿生长正常组(P<0.05)。Hcy、VEGF联合检测胎盘功能、胎儿生长受限的AUC分别为0.896、0.916,均显著高于Hcy、VEGF单独检测(P<0.05)。结论:血清Hcy、VEGF在重度PE患者胎盘功能以及胎儿生长受限评估中具有重要作用,可预测胎盘功能以及胎儿生长受限情况。 Objective:To investigate the serum Homocysteine(Hcy)and Vascular endothelial growth factor in patients with severe Preeclampsia(PE).The predictive value of VEGF levels in placental function and fetal growth restriction.Methods:105 patients with severe PE admitted to the Department of Obstetrics and Gynecology of our hospital from July 2020 to July 2024 were selected as the study objects.The placental function and fetal growth restriction in the third trimester of severe PE were analyzed,and the patients were divided into normal placental function group and abnormal placental function group.Fetal growth restriction group and fetal growth normal group.Serum Hcy and VEGF levels in the normal placental function group and the abnormal placental function group,and the serum Hcy and VEGF levels in the fetal growth restriction group and the normal fetal growth group were detected and compared respectively.ROC characteristic curve was drawn to calculate the predictive value of Hcy and VEGF alone and combined detection on placental function and fetal growth restriction.Results:Among 105 patients with severe PE,74 patients with abnormal placental function were included in the abnormal placental function group and 31 patients with normal placental function were included in the normal placental function group.Among the 105 patients with severe PE,54 patients with fetal growth restriction were included in the fetal growth restriction group,and the other 51 patients with normal fetal growth were included in the normal fetal growth group.The serum Hcy level in the abnormal placental function group was significantly higher than that in the normal placental function group,and the serum Hcy level in the fetal growth restriction group was significantly higher than that in the normal fetal growth group(P<0.05).The serum VEGF level in the abnormal placental function group was significantly lower than that in the normal placental function group,and the serum VEGF level in the fetal growth restriction group was significantly lower than that in the normal fetal growth group(P<0.05).The AUC of placental function and fetal growth restriction measured by Hcy and VEGF combined were 0.896 and 0.916,respectively,which were significantly higher than that by Hcy and VEGF alone(P<0.05).Conclusion:Serum Hcy and VEGF play an important role in evaluating placental function and fetal growth restriction in patients with severe PE,and can predict placental function and fetal growth restriction.
